Abstract

Namen diplomske naloge je določiti genotipe specifičnih enonukleotidnih polimorfizmov tarčnih genov, ki so odgovorni za višji ali nižji začetni odmerek varfarina. To so VKORC1-1639G>A, CYP2C92-430C>T, CYP2C93-1075A>C, CYP4F2-132C>T. Varfarin je učinkovina, oralni antikoagulant, ki se predpisuje bolnikom z pljučno embolijo, globoko vensko trombozo, tromboembolijo. Varfarinska terapija zahteva veliki nadzor nad pravilnim odmerkom varfarina, zdravljenje je dolgotrajno in ima neželene stranske učinke kot so krvavitve. Na optimalni odmerek varfarina v kar 40% vplivajo genetski dejavniki, zato je smiselna genotipizacija omenjenih tarčnih genov. Uporabili smo alelno specifično verižno reakcijo s polimerazo in nato gelsko elektroforezo. Metoda je hitra, enostavna, ekonomsko učinkovita.Rezultati diplomske naloge kažejo, da je 87% ljudi takšnih pri katerih lahko pričakujemo po zauzetju normalnega odmerka varfarina pretiran odziv.

Secondary abstract: The purpose of this thesis is to determine the genotypes of specific nucleotide polymorphisms target genes that are responsible for higher or lower initiating dose of warfarin. These are the VKORC1-1639G> A, CYP2C92-430C> T, CYP2C93-1075> C, CYP4F2-132C> T. Warfarin is the active ingredient, oral anticoagulant, which is prescribed to patients with pulmonary embolism, deep venous thrombosis, thromboembolism. Warfarin therapy requires great control of the correct dose of warfarin, treatment is long and has side effects such as bleeding. At the optimal dose of warfarin in 40% affected by genetic factors, so it makes sense genotyping of these target genes. We used allele-specific PCR and then gel electrophoresis. The method is fast, simple, economic.Results show that 87% of these people are those, where we can expect after warfarin take excessive response
