Abstract
Uvod: Medicinska sestra potrebuje široko strokovno znanje na področju psihiatrije ter empatije, da lahko pacienta z duševno motnjo razume in mu ustrezno svetuje. Ko pacient zapusti bolnišnico, nadaljuje z zdravljenjem v domačem okolju. V skupnostni obravnavi sodelujejo medicinska sestra, psihiater, socialni delavec, psiholog in delovni terapevt. Pri obravnavi je poudarek na samostojnosti pacienta in izobraževanju njega in svojcev. Namen: Namen diplomskega dela je povečati znanje o skupnostni psihiatrični obravnavi pri pacientu s hudo in ponavljajočo se duševno motnjo, ter prikazati najpogostejše intervencije medicinske sestre. Metode dela: Uporabili smo deskriptivno metodo s pregledom tuje in domače literature. Iskanje je potekalo s pomočjo podatkovnih baz, kot sta Cinahl in PubMed, svetovnem spletu ter v knjižnici Zdravstvene fakultete Ljubljana. Uporabili smo literaturo, ki je bila objavljena od leta 2006 do 2016 in je dostopno celotno besedilo. Rezultati: Rezultati raziskav so pokazali izboljšanje duševnega zdravja pacientov, ki so bili vključeni v skupnostno obravnavo. Zmanjšale so se ponavljajoče se hospitalizacije, število bolnišničnih dni, uživanje prepovedanih substanc. Napredek je bil viden pri redni uporabi predpisane terapije, boljši socialni vključenosti in samostojnosti pri vsakdanjem življenju. Pacienti in svojci so izkazali zadovoljstvo z vlogo medicinske sestre v skupnostni psihiatrični obravnavi pri pacientu s hudo in ponavljajočo se duševno motnjo. Razprava in zaključek: Pri skupnostni obravnavi oseb s hudo in ponavljajočo se duševno motnjo je potrebno sodelovanje in zaupanje pacientov. Multidisciplinarni tim deluje na principu čim hitrejše samostojnosti pacientov.

Introduction: Nurse needs sufficient expertise in the field of psychiatry and empathy to be patient and to understand patients medical condition so she can advice him properly. When the patients leave the hospital, treatment is continued at home. Nurse in community works in cooperation with psychiatrist, social worker, psychologist and occupational therapist. The community treatment must be focused on the autonomy of the patients, and the education of thereof and relatives. Purpose: The purpose of this thesis is to increase knowledge of community treatment with patients with severe and repeated mental disorder, and to show the most common inteventions of nurses. Methods: We used descriptive method with scrutiny domestic and foreign literature. Search is carried out by means of databases such as CINAHL and PubMed, the internet, and the Faculty of Health Sciences library. We used a literature that has been published since 2006 - 2016 which is accessible throughout. Results: The results of research have shown improved mental health in patients who were involved in community treatment. The number of re-hospitalizations and hospital days was reduced, just as the use of prohibited substances. Regular use of the prescribed therapy shows better social inclusion and independence in everyday life. Patients and relatives have demonstrated satisfaction with role of nurses in community psychiatric treatment with patients with severe and repeated mental disorder. Discussion and conclusion: The community treatment of patients with severe and repeated mental disorder requires cooperation and trust of patients. Multidisciplinary team should work on the principle of the fastest possible independence of the patients. |
