Abstract
Uvod: Fizioterapija pri otrocih s cerebralno paralizo je pogosto usmerjena v izboljšanje sposobnosti hoje in vadbo ravnotežja. Za ocenjevanje otrok na teh področjih sta na voljo 6-minutni test hoje in časovno merjeni test vstani in pojdi. Njune merske lastnosti pri ocenjevanju otrok so ugotavljali v več raziskavah. Namen: Namen diplomskega dela je bil pregledati izsledke raziskav o veljavnosti in zanesljivosti 6-minutnega testa hoje in časovno merjenega testa vstani in pojdi pri otrocih s cerebralno paralizo. Metode: Raziskovalne članke v angleščini smo iskali v podatkovnih zbirkah MEDLINE, CINAHL, Cochrane Library, ScienceDirect in PeDRO. Uporabili smo iskalne kombinacije ključnih besed »children«, »cerebral palsy«, »CP«, »gait«, »walk«, »balance«, »6-minute walk test«, »timed up & go«, »psychometric«, »valid« in »reliab«. Dodatno smo pregledali še arhiva revij Fizioterapija in Rehabilitacija. Rezultati: V pregled literature smo vključili štiri članke za 6-minutni test hoje in šest člankov za časovno merjeni test vstani in pojdi. Za 6-minutni test hoje so v treh raziskavah poročali o zelo visoki (ICC = 0,98 oz. ρ = 0,95), v eni pa o visoki (ICC = 0,80) zanesljivosti. V eni raziskavi so ugotovili konstruktno in v eni konvergentno veljavnost testa. Za časovno merjeni test vstani in pojdi so v vseh šestih raziskavah ugotovili zelo visoko zanesljivost (ICC < 0,94). V štirih raziskavah so poročali o konstruktni in v dveh o konvergentni veljavnost testa. Razprava in zaključek: Oba testa sta zanesljiva in veljavna za ocenjevanje otrok s cerebralno paralizo, starih od 2 do 18 let in stopenj I do III po sistemu za razvrščanje glede na njihove grobe gibalne funkcije. Vse raziskave so bile narejene na majhnem vzorcu otrok in z različno izvedbo testov, kar omeji njihovo primerljivost. Za ugotavljanje vpliva spola, starosti, antropometričnih in kognitivnih značilnosti otrok na zanesljivost in veljavnost testov so potrebne dodatne raziskave.

Validity and reliability of the 6-minute walk test and the timed up and go test in children with cerebral palsy - literature review |

Introduction: Physiotherapy interventions in children with cerebral palsy often focus on improving their walking ability and dynamic balance. The 6-minute walk test and timed up and go test are available to measure their performance in these two fields. Numerous studies have researched their psychometric properties. Purpose: The purpose was to review the results of the studies which researched reliability and validity of the 6-minute walk test and timed up and go test in children with cerebral palsy. Methods: The used working method was a literature review. English literature has been searched in the MEDLINE, CINAHL, Cochrane library, ScienceDirect and PeDRO scientific databases. The key phrases children, cerebral palsy, CP, gait, walk, balance, 6-minute walk test, timed up & go, psychometric, valid and reliab were used. The archives of publications Fizioterapija and Rehabilitacija were also searched to identify the additional resources in Slovene language. Results: The search yielded four relevant articles for the 6-minute walk test and six for the timed up and go test. Authors of three studies reported very high (ICC = 0,98 or ρ = 0,95) and another one high (ICC = 0,80) reliability of the 6-minute walk test. One study showed construct and one convergent validity of the test. Very high (ICC < 0,94) reliability was reported in all of the six studies for the timed up and go test. Construct validity was confirmed in four and convergent validity in two of them. Discussion and conclusion: Both test are reliable and valid for assessing children with cerebral palsy, between 2 and 18 years old and classified as level I to III according to Gross motor function classification system. The reviewed studies were all conducted on small and heterogeneous samples, which limit the comparison between them. Further researches are needed to evaluate the impact of sex, age, anthropometric and cognitive characteristics on reliability and validity of the tests. |
