Abstract

Uvod: Bolečine v ramenskem sklepu so pogost pojav z incidenco 9,5 na 1000 pacientov v primarni zdravstveni oskrbi in s točkovno prevalenco od 7 do 26 %. Večina pacientov z bolečinami v ramenskem sklepu kaže klinične simptome utesnitvenega sindroma ramenskega sklepa. Izraz zajema različne stopnje poškodbe, od zgodnjega vnetja kit mišic rotatorne manšete pa vse do delnih oz. celo popolnih pretrganj. Pri določanju optimalnega zdravljenja se mnenja razhajajo. Poleg operativnega zdravljenja in injiciranja kortikosteroidov se uporablja še ultrazvočna terapija, terapija z visokointenzivnim laserjem, terapija z udarnimi globinskimi valovi in manualna terapija. Namen: Namen diplomskega dela je opisati učinkovitost manualne terapije pri konzervativnem zdravljenju pacientov z utesnitvenim sindromom ramenskega sklepa. Metode dela: V podatkovnih bazah, dostopnih na internetu, smo izbrali in pregledali šest randomiziranih kontroliranih študij, ki so ustrezale vključitvenim kriterijem. Rezultati: Raziskave so primerjale učinke manualne terapije z drugimi metodami zdravljenja. V vseh raziskavah so z uporabo manualne terapije dosegli izboljšanje pacientovega stanja, vendar so statistično pomembne razlike v primerjavi z drugimi oblikami zdravljenja ugotovili le v eni od vključenih raziskav. Razprava in zaključek: Glede na rezultate raziskav ne moremo trditi, da je obravnava utesnitvenega sindroma le z manualnimi tehnikami najbolj učinkovita. Omeniti moramo, da so v vseh raziskavah poleg manualne terapije pacienti bili deležni tudi kinezioterapevtkse obravnave v obliki vadbenega programa. Tako lahko trdimo, da je manualna terapija učinkovit element multidisciplinarne obravnave utesnitvenega sindroma ramenskega sklepa.

Secondary abstract: Introduction: Pain in the shoulder joint is a common disorder with an incidence of 9.5 per 1000 patients in primary health care with a point prevalence of 7 to 26 %. Most patients with pain in the shoulder joint show clinical symptoms of shoulder impingement syndrome. The term covers various degrees of damage, from early inflammation of tendons to partial or even complete tears. When determining optimal treatment, opinions differ. In addition to surgical treatment and injection of corticosteroids, ultrasound therapy, high intensity laser therapy, therapy with shock waves and manual therapy are also used. Purpose: The purpose of the thesis is to describe the effectiveness of manual therapy in the conservative treatment of shoulder impingement syndrome. Methods: In databases accessible on the web, we selected an overview of six randomized controlled studies that fited the inclusion criteria. Results: The studies compared the effects of manual therapy with other treatment methods. In all studies, the use of manual therapy resulted in an improvement of the condition, but statistically significant differences compared to other forms of treatment were found only in one of the studies reviewed. Discussion and conclusion: According to the results of the research, it cannot be said that the treatment of shoulder impingement syndrome with manual techniques is the most effective treatment method. It should be noted that in all the studies, in addition to the manual therapy, patients were also treated with kinesiotherapy treatment in the form of a training program. Thus, it can be argued that manual therapy is an effective element of the multidisciplinary treatment of the shoulder joint.
