Abstract

Uvod: Skupnostna psihiatrična obravnava pomeni obravnavo osebe z duševno motnjo pretežno na njegovem domu, v krogu njegove socialne mreže, saj so osebe s tovrstnimi težavami običajno doma in ne poiščejo strokovne pomoči ali pa jo poiščejo prepozno. V skupnostni psihiatrični obravnavi ima pomembno vlogo predvsem diplomirana medicinska sestra. Namen: Namen diplomskega dela je bil, na podlagi pridobljenih podatkov iz negovalne dokumentacije, ugotoviti pri katerih življenjskih aktivnostih so najpogosteje zaznane težave in v kakšni obliki se pojavljajo ter glede na težave opredeliti pomen vključevanja diplomirane medicinske sestre v proces obravnave. Metode dela: Empirični del diplomskega dela, ki je bil opravljen v zdravstvenem domu Sevnica, je vključeval pregled negovalne dokumentacije uporabnikov in pridobitev podatkov iz računalniškega sistema. Podatki so bili izpisani v opazovalni list (priloga). Raziskovalni vzorec je obsegal 176 uporabnikov, ki so bili v skupnostno psihiatrično obravnavo vključeni v letu 2017. Za raziskovanje je bilo pridobljeno dovoljenje vodstva Zdravstvenega doma Sevnica, privolitev etične komisije pa ni bila potrebna, saj je šlo za pregled podatkov zapisanih v fizični ali elektronski dokumentaciji. Rezultati: V letu 2017 je služba skupnostne psihiatrične obravnave za Posavje obravnavala 176 oseb, med katerimi je s 63 % prevladovala medicinska diagnoza shizofrenija. Povprečna starost uporabnikov je bila 55,8 let. Iz zapisov obravnav je bilo ugotovljeno največ odstopanj na področju temeljnih življenjskih aktivnostih prehranjevanje in pitje, spanje in počitek, osebna higiena in urejenost, izogibanje nevarnostim v okolju ter učenje in pridobivanje znanja o zdravju. Razprava in zaključek: Uporabniki skupnostne psihiatrične obravnave imajo pogosto prisotne težave na področju večine življenjskih aktivnosti, zato ima medicinska sestra pomembno vlogo pri dvigovanju samostojnosti uporabnika, pri vključevanju drugih služb v obravnavo le tega ter sprotnemu ocenjevanju njegovega napredka. Za uspešno koordinacijo delovanja multidiciplinarnega tima pa potrebuje dodatna znanja pridobljena na formalnih in neformalnih izobraževanjih.

Secondary abstract: Introduction: Community mental health care refers to the treatment of a person with a mental disorder predominantly at his or her home in the circle of his/her social network. People with such problems are usually at home and do not seek professional help or they find it too late. In a community mental health care, a registered nurse plays an important role. Purpose: The purpose of diploma work was to present the structure of the users of the community mental health care service of Posavje, which operates in the Sevnica Health Center. The goal was also to determine which life activities are the most commonly perceived, what kind of problems users face and in what form they appear. In the end diploma work is evaluates the needs for nursing care and the importance of involving the registered nurse in the process of community mental health treatment. The data was acquired from the nursing documentation of the users. Methods of work: The research carried out at the Sevnica Health Center included a review of users' nursing documentation and the acquisition of data from the computer system. The data were displayed in the observation sheet (annex). The research sample consisted of 176 users who were included in community mental health treatment in 2017. The permission of the management of Sevnica Health Center was obtained for the research, and the consent of the ethics commission was not necessary as it was a review of the data recorded in the physical or electronic documentation. Results: In 2017, the community mental health treatment service for Posavje was treated by 176 people, of whom 63 % had the medical diagnosis of schizophrenia. The average age of users was 55,8 years. From the records of the hearings, most deviations were found in the field of basic life activities of eating and drinking, sleeping and resting, personal hygiene and orderliness, avoiding environmental hazards, and learning and acquiring health knowledge. Discussion and conclusion: Community mental health treatment users often have problems in most life activities, so the registered nurse plays an important role in raising the user's autonomy, in involving other services in dealing with it, and in the ongoing assessment of his progress. In order to successfully coordinate the work of the multidisciplinary team, however, additional knowledge is needed in formal and non-formal education.
