Abstract

Cilj doktorske disertacije je bila analiza učinka 9-tedenskega zgodnjega rehabilitacijskega vadbenega programa s pričetkom po tretjem tednu po posegu zatrditve ledvene hrbtenice. Vadbeni protokol temelji na načelih vadbe za moč in je testiran z objektivnimi parametri. V raziskavi je sodelovalo 34 merjencev (19 moških in 15 žensk), starih med 45 in 70 let, ki so bili predhodno določeni za poseg zatrditve ledvene hrbtenice. Za razdelitev pacientov v skupine je bila uporabljena metoda prikritega dodeljevanja brez blokiranja. Eksperimentalna skupina je pričela z vadbenim protokolom tri tedne po posegu. Merjenci so v obdobju devetih tednov vadili dvakrat tedensko. Osnovni namen vadbe je bil aktivacija stabilizacijskih mišic hrbtenice. Kontrolna skupina ni izvajala dodatne vadbe. Testiranje bolečine in funkcionalnih sposobnosti je bilo izvedeno pred vadbenim protokolom, t. j. tri tedne po operativnem posegu, in po vadbenem protokolu, t. j. tri mesece po operativnem posegu. Po opravljeni vadbi v eksperimentalni skupini ni bilo težav z zatrditvijo vijakov. Pri obeh skupinah je bilo opazno izboljšanje prehojene razdalje pri 6-minutnem testu hoje po treh mesecih po posegu. Za razliko od kontrolne skupine je bilo v eksperimentalni skupini statistično značilno izboljšanje rezultatov testa dosežne višine, časovnega zamika lateralne sile trupa glede na pričetek akcije ter testov moči stabilizatorjev trupa, razen upogiba trupa. Prav tako je bilo v eksperimentalni skupini značilno izboljšanje vseh meritev največjega hotenega izometričnega navora, dosežne višine in časovnega zamika lateralne sile trupa in pričetka akcije. Ugotovitve kažejo, da je 9-tedenski vadbeni program zgodnje rehabilitacije, ki temelji na osnovah načel vadbe za moč, varen že po treh tednih po posegu zatrditve ledvene hrbtenice. Znotraj dvomesečnega raziskovanega obdobja hkrati omogoča značilno izboljšanje določenih funkcionalnih sposobnosti.

Secondary title: The effects of early rehabilitation on clinical results and biomechanical parameters after instrumental lumbar spine fusion
Secondary abstract: The aim of the study was to analyse the safety and effects of early initiation of a 9-week rehabilitation protocol including objective measurement outcomes after lumbar spine fusion based on principles of strength training. The study recruited 34 patients (19 men & 15 women), aged from 45 to 70 years, who had undergone lumbar spine fusion. The method of concealed random allocation without blocking was used to form two groups. The strength training group started rehabilitation 3 weeks after the surgery. Patients exercised twice per week over the course of 9 weeks focusing on muscle activation of lumbopelvic stabilization muscles. The control group followed a standard postoperative protocol, where no exercises were performed at that stage of rehabilitation. Functional outcomes and plain radiographs were evaluated at 3 weeks and subsequently at 3 months after the surgery. Neither hardware loosening nor failure was observed in the training group. Both groups improved their walking speed after 3 months, although improvement in the training group was significantly greater than that of the control group. Moreover, the training group significantly improved after the training period in all isometric trunk muscle measurements, standing reach height and pre-activation pattern. The study showed that early initiation of a postoperative rehabilitation program based on principles of strength training is safe, three weeks after lumbar spine fusion and enables earlier functional recovery than standard rehabilitation protocol.
