Abstract
Teoretična izhodišča: Paliativna oskrba je celostna oskrba pacienta s kronično nenalezljivo boleznijo, ki se osredotoča na obvladovanje bolečine in drugih spremljajočih motečih simptomov ter lajšanje psihičnih, socialnih in duhovnih težav pacienta in njegovih svojcev. Namen zaključnega dela je s sistematičnim pregledom in analizo literature raziskati ovire ter težave, s katerimi se srečujejo medicinske sestre ob obravnavi posameznikov v paliativni oskrbi.
Metodologija: Uporabljen je bil sistematični pregled literature. V pregled so bile vključene naslednje baze: Cobbiss, PubMed, CINAHL in ScienceDirect.. Iskanje literature je potekalo od decembra 2017 do junija 2018. Iz iskalnega nabora 1467 zadetkov je bilo v končno analizo vključenih 14 člankov. Izvedena je bila vsebinska analiza člankov.
Rezultati: Ugotovljenih je bilo veliko ovir za vključitev paliativne oskrbe katere smo združili v podteme in v tri glavne teme: ovire s strani medicinskih sester, ovire s strani pacienta in ovire v komunikaciji. Ovire, ki se pojavljajo v domeni medicinskih sester ter tudi pacientov in svojcev, so predvsem strah, ne sprejemanje in neznanje oziroma nepoznavanje paliativne oskrbe. V temi komunikacija pa se pojavijo logistične in kulturne ovire ter prav tako strah.
Diskusija in zaključek: Zgodnjo vključitev paliativne oskrbe zavira mnogo dejavnikov, pa vendar je možno vzroke odpravit. Potrebno je izobraževanje medicinskih sester na področju paliativne oskrbe in tudi komunikacije s pacientom in svojcem ter tudi izobraževanje laične populacije za lažje sprejemanje pomoči v obliki paliativne oskrbe.

The most common nurse-perceived barriers in palliative care |

Introduction: Palliative care is a holistic care of a patient with a chronic non-infectious disease that focuses on coping with pain and other accompanying disturbing symptoms and relieving the psychological, social and spiritual problems of the patient and his family. The purpose of this work is to examine barriers and problems faced by nurses when examining individuals in palliative care through systematic review and analysis of literature.
Methodology: A systematic review of literature was used. The following bases were included in the review: Cobbiss, PubMed, CINAHL and Science Direct. The literature search took place from December 2017 to June 2018. Out of 1467 hits, 14 articles were included in the final analysis. A substantive article analysis was carried out.
Results: There were many obstacles to implementation of palliative care, which we combined into subtopics and three main themes: barriers by nurses, patient’s barriers and communication barriers. Obstacles appearing in the domain by nurses, as well as patients and relatives, are mainly fear, possible rejection and ignorance or not enough knowledge about palliative care. In theme communication, there are logistical and cultural barriers and fear.
Discussion and Conclusion: Early implementation of palliative care is prevented by many factors; however, the causes can be eliminated. Nursing education in the field of palliative care, as well as communication with the patient and family. Also, education of population is required to facilitate the reception of help in the form of palliative care. |
