Abstract

Uvod: Lateralni epikondilitis je degeneracijsko stanje, ki na lateralni strani epikondila humerusa vpliva na izvor skupne tetive ekstenzornih mišic zapestja, posebno na m. ekstensor carpi radialis brevis. Glavni simptom je bolečina. Poznamo farmakološko, kirurško in konzervativno obliko zdravljenja. Zdravljenje z udarnimi globinskimi valovi je fizioterapevtska metoda, ki spada v konzervativno obliko zdravljenja in se že vrsto let uporablja pri obravnavi različnih mišično skeletnih stanjih. Namen: Namen diplomskega dela je s pomočjo pregleda literature predstaviti učinke terapije z udarnimi globinskimi valovi na lateralni epikondilitis oz. ugotoviti, kateri parametri terapije so najprimernejši pri obravnavi lateralnega epikondilitisa. Metode dela: Za izdelavo diplomskega dela je bila izbrana deskriptivna oz. opisna metoda s pregledom literature, ki je bil opravljen v elektronskih podatkovnih zbirkah PubMED, PEDro Cochrane Library, CINAHL in Science Direct. Rezultati: Po pregledu vključitvenih in izključitvenih kriterijev je bilo v pregled literature vključenih devet kontroliranih randomiziranih raziskav, ki so terapijo z udarnimi globinskimi valovi primerjale z različnimi fizioterapevtskimi in farmakološkimi oblikami zdravljenja. Avtorji so različnih časovnih obdobjih ocenjevali bolečino, mišično zmogljivost roke, funkcijo zgornjega uda in zadovoljstvo pacienta. Razprava in zaključek: V petih raziskavah so avtorji ugotovili učinkovitost terapije z udarnimi globinskimi valovi pri zmanjševanu bolečine, izboljšanju grobe mišične zmogljivosti, funkcije zgornjega uda in pacientovega zadovoljstva. V primerjavi s farmakološko obliko zdravljenja so avtorji ugotovili dolgoročni učinek, v primerjavi z drugimi fizioterapevtskimi metodami pa se je terapija z udarnimi globinskimi valovi izkazala za učinkovitejšo v vseh obdobjih merjenja. Parametri terapij so se med raziskavami razlikovali, nekateri avtorji so vključili še druge intervencijske postopke, ki so lahko vplivali na končni rezultat. Po pregledu literature ne moremo trditi, da je terapija z udarnimi globinskimi valovi najučinkovitejša obravnava lateralnega epikondilitisa, zato bi bilo potrebno izvesti dodatne raziskave, ki bi potrjevale njen učinek in ugotovile najprimernejše parametre terapije.

Secondary title: Effects of treatment of lateral epicondylitis with extracorporeal shockwave therapy
Secondary abstract: Introduction: Lateral epicondylitis is a degenerative condition that affects the origin of the common tendon of the extensor muscles of the wrist located at the lateral epicondyle of the humerus, especially on m. extensor carpi radialis brevis. The main symptom is pain. There are three known forms of treatment: pharmacological, surgical and conservative. Extracorporeal shock wave therapy is a physiotherapeutic method, classified as a conservative form of treatment and has been used for many years as a treatment of various musculoskeletal conditions. Purpose: The purpose of the diploma is to present the effects of extracorporeal shock wave therapy on the lateral epicondylitis and to determine which parameters of therapy are the most appropriate in the treatment of lateral epicondylitis. Methods: For the preparation of the diploma the descriptive method with a literature review was used. Search of studies was performed in databases PubMED, PEDro Cochrane Library, CINAHL and Science Direct. Results: After reviewing the literature, nine studies met the inclusion and exclusion criteria. Studies compared extracorporeal shock wave therapy to various physiotherapeutic and pharmacological forms of treatment. The authors assess the pain, muscular strength, upper limb function and patient satisfaction at different time periods. Discussion and conclusion: In five studies, the authors report the effectiveness of extracorporeal shock wave therapy in reducing pain, improvements in muscle strength, upper-limb functions, and patient satisfaction. When extracorporeal shock wave therapy is compared with the pharmacological form of treatment, the authors find long-term effects, and in comparison, with other physiotherapeutic methods, the extracorporeal shock wave therapy turns out to be superior in all measurement periods. The therapeutic parameters vary in various studies, additionaly; some authors also include other intervention procedures that could affect the outcome. After literature review, we can assume that extracorporeal shock wave therapy shall not be the main method when treating lateral epicondylitis, furthermore, additional studies are needed to confirm its effect and determine the most optimal parameters of therapy.
