Abstract
Demografske spremembe pomembno vplivajo na javne izdatke za zdravstvo in dolgotrajno oskrbo. V prihodnosti se pričakuje spreminjanje strukture prebivalstva, in sicer večanje deleža starejših od 85 let ter manjšanje deleža delovno aktivnega prebivalstva. Tveganje za potrebo po dolgotrajni oskrbi se povečuje s starostjo, zato bodo demografske spremembe vplivale tudi na povečanje obsega storitev dolgotrajne oskrbe in z njo povezanih izdatkov. Sistemi dolgotrajne oskrbe so v vsaki državi urejeni drugače. Z reformami se države poskušajo prilagoditi potrebam starajoče se populacije. Reforme zajemajo predvsem razvoj na področju zdravstva in dolgotrajne oskrbe ter njuno financiranje in izboljšanje kadrovskega potenciala za nove potrebe prebivalstva.
Na zdravje ljudi in posledično na potrebo po dolgotrajni oskrbi pomembno vpliva tudi preventiva. Tako primarna kot sekundarna preventiva sta ključnega pomena za zdravje ljudi in zmanjšanje izdatkov za zdravstvo in dolgotrajno oskrbo. Uvedba teh programov po letu 2010 v Sloveniji (npr. Zora, Dora, Svit) že kaže rezultate pri zmanjšanju obolevnosti in umrljivosti prebivalstva. Prav tako pa ne smemo zanemariti pomembnosti preventive na področju zdrave prehrane, gibanja, alkohola, kajenja in duševnega zdravja. V Evropi je visok odstotek uživanja alkohola, kar pušča posledice na zdravju. Prav tako na zdravje vpliva slog posameznika tako v mladosti kot v starejši dobi življenja. Kljub pomembnosti preventive države še vedno več vlagajo v storitve kurativne dejavnosti kot v storitve preventivne dejavnosti. Zaradi tega so izdatki za storitve kurative bistveno večji kot pri preventivi. Za izboljšanje zdravstvenega stanja prebivalstva ter zmanjšanje vseh izdatkov v okviru zdravstva in dolgotrajne oskrbe bi bilo potrebno povečati vlogo preventive.

Demographics changes and control of long-term care expenditures |
Secondary abstract: |
Demographic changes importantly influence public expenditure for health care and long-term care. In the future, changes of the population structure are expected in direction to increasing number of elderly over 85 and lowering of the working force population. The risk for long-term care need increases with age, therefore, demographic changes will influence increasing of services for long-term care and its expenses. Systems for long-term care are arranged differently in every state. The states try to adapt to the needs of the ageing population with reforms. The reforms especially include the developmetn of health and long-term care and their financing and improvement of the cadre potential fort he population's new needs.
Prevention importantly influences health of people and consequentially the need for long-term care. Primary as well as secondary prevention are of key importance for people's health and lowering of the health and long-term care expenses. Introduction of the programmes after 2010 in Slovenia (e.g. Zora, Dora, Svit) shows results in lowering of illness rate and mortality of population. We also cannot neglect the importance of healthy diet, physical activity, alchohol, smoking, and mental health prevention. Health is also influenced by individual life style in young age and when people are older. Even though prevention is important, states invest more and more into services of curative activities than prevention activities. Therefore, the expenses for curative are essentially higher than prevention. The role of prevention should be increased to imporve health state of population and lower the expenses of health care and long-term care. |
