Abstract
Uvod: Torakalna drenaža je kirurški poseg, s katerim se odvaja zrak in/ali tekočino iz plevralnega prostora. Na ta način se olajša oteženo dihanje, kašelj in bolečina na prizadeti strani prsnega koša. V primerih, ko je potrebno dolgotrajno izvajanje dreniranja, se uporabljajo sistemi za drenažo v domačem okolju. Patronažna medicinska sestra ima pomembno vlogo v celostni zdravstveni oskrbi teh pacientov. Namen: Namen diplomskega dela je predstaviti zdravstveno nego pacienta z vstavljenim Pleurx® drenažnim sistemom v domačem okolju. Cilja diplomskega dela sta: predstaviti raziskave o uporabi Pleurx® drenažnih sistemov v domačem okolju ter oblikovati navodila za izvedbo te oblike dreniranja za patronažne medicinske sestre. Metode dela: Uporabljena je bila deskriptivna metoda dela s pregledom tuje in domače strokovne in znanstvene literature. Literatura je bila iskana s pomočjo digitalne knjižnice Univerze v Ljubljani v podatkovnih bazah CINAHL in Medline in preko kataloga slovenskih knjižnic COBIB.SI. Uporabljene so bile naslednje ključne besede: Pleurx®, protokol, smernice, patronažna medicinska sestra, plevralna drenaža, raziskave in navodila za delo. Uporabljena je prosto dostopna literatura, ki je bila objavljena od leta 2006 do leta 2019. Rezultati: Raziskav o uporabi sistemov za torakalno drenažo v domačem okolju je malo. Raziskovalci ugotavljajo, da so Pleurx® drenažni sistemi optimalna rešitev pri paliativni obravnavi pacientov s plevralnim izlivom, ki imajo zaradi malignega obolenja ali srčnega popuščanja krajšo pričakovano življenjsko dobo. Patronažne medicinske sestre potrebujejo jasna navodila za izvedbo dreniranja, zato so na podlagi pregleda literature oblikovana navodila, ki izvedbo prikazujejo po posameznih zaporednih korakih in so prilagojena izvajanju dreniranja v domačem okolju. Razprava in zaključek: Pleurx® katetri učinkovito zmanjšajo simptome in znake bolezni ter pogostost ponovnih hospitalizacij pri paliativnih pacientih z različnimi vzroki za kronični plevralni izliv. Patronažna medicinska sestra je tem pacientom in njihovim bližnjim v oporo pri reševanju težav, ki so povezane z izvajanjem dreniranja v domačem okolju. Patronažna medicinska sestra mora slediti najnovejšim smernicam in navodilom za delo in svoje aktivnosti individualno prilagoditi vsakemu pacientu posebej. Pri njenem delu so ji lahko v pomoč v diplomskem delu oblikovana navodila za delo, v katerih so tudi odgovori na nekatere dileme in vprašanja o optimalni izvedbi posega v domačem okolju.

Introduction: Toracal drainage is a surgical procedure that removes the air and / or the fluid from the pleura, so we alleviate breathing, coughing and pain on the affected side of the chest. In cases where long-term drainage is required, drainage systems in the domestic environment are used. The community nurse plays an important role in the overall health care of these patients. Purpose: The purpose of the diploma work is to present the patient's health care with the inserted Pleurx® drainage system in the home environment. The aim of the diploma work is to present research on the use of Pleurx® drainage systems in the home environment and to develop instructions for the implementation of this form of drainage for community nurses. Methods: A descriptive method of work was used with an overview of foreign and domestic professional and scientific literature. Literature was searched with the digital library of the University of Ljubljana in the CINAHL and Medline databases and through the catalog of Slovenian libraries COBIB.SI. The following keywords were used: Pleurx®, protocol, guidelines, community nurse, pleural drainage, research and work instructions. Free accessed literature was selected, which was published from 2006 to 2019. Results: There is little research on the use of toracial drainage systems in the home environment. Researchers have found that Pleurx® drainage systems are the optimal solution for palliative treatment of patients with pleural effusion, which have a shorter life expectancy due to malignancy or heart failure. Community nurses need clear instructions for drainage, so instructions are drawn up based on the literature review, which show the performance by individual successive steps and are adapted to the implementation of drainage in the home environment. Discussion and conclusion: Pleurx® catheters effectively reduce the symptoms and signs of the disease and the frequency of re-hospitalization in palliative patients with various causes of chronic pleural effusion. The community nurse supports these patients and their close relatives in solving problems related to the implementation of drainage in the home environment. The community nurse must follow the latest guidelines and instructions to work and adapt to each patient individually. The diploma thesis can help her with work instructions, which also provide answers to some dilemmas and questions about the optimal implementation of the intervention in the home environment. |
