Abstract

Uvod: Kronična bolečina predstavlja pomemben socialni in ekonomski problem, ki neposredno prizadene 630 milijonov ljudi po vsem svetu, prevalenca pa se je od leta 1992 do 2006 skoraj potrojila. V slovenščino imamo do sedaj prevedena dva vprašalnika za ocenjevanje bolečine v hrbtu, nismo pa imeli prevedenega nobenega od vprašalnikov za ugotavljanje tveganja za razvoj kronične bolečine. Namen: Primarni namen magistrskega dela je bil prevesti Presejalni vprašalnik Keele STarT Back za paciente z bolečino v hrbtu v slovenski jezik, preveriti zanesljivost njegovega ponovljenega izpolnjevanja in preveriti učinka stropa in tal slovenskega prevoda. Sekundarni namen je bil oblikovati priporočila za paciente, ki vsebujejo ključne informacije o BSH in seznam postopkov za samoobvladovanje bolečine. Metode dela: Presejalni vprašalnik Keele STarT Back za paciente z bolečino v hrbtu smo po standardnem protokolu prevedli iz angleškega v slovenski jezik in nazaj. Zanesljivost ponovljenega ocenjevanja smo izračunali z metodo specifičnega ujemanja, vsebinsko veljavnost pa z učinkoma stropa in tal. Na podlagi pregleda strokovne literature smo oblikovali kratka pojasnila glede narave bolečine v hrbtu in priporočila za njeno samoobvladovanje. Rezultati: V raziskavo je bilo skupno vključenih 42 pacientov. Na podlagi presejalnega vprašalnika jih ima 42,9 % nizko tveganje, 33,3 % srednje in 23,8 % visoko tveganje za pojav kronične bolečine. Z vidika zanesljivosti ima test odlično specifično ujemanje med prvim in ponovljenim ocenjevanjem, in sicer 91,4 % za skupino z nizkim, 85,7 % s srednjim in 95,2 % z visokim tveganjem. Ugotovili smo dobro vsebinsko veljavnost, saj test nima učinkov stropa in tal. Razprava in zaključek: Ugotovili smo, da ima Presejalni vprašalnik Keele STarT BacK za paciente z bolečino v hrbtu odlično zanesljivost ponovljenega ocenjevanja ter nima učinkov stropa in tal. Uporaba vprašalnika v klinični praksi predstavlja pomemben prispevek k vzpostavitvi sistemske rešitve v slovenskem prostoru glede obravnave pacientov z BSH in zmanjševanja odsotnosti z dela zaradi BSH, saj bodo s kakovostnim razvrščanjem pacientov ti hitreje deležni ustrezne obravnave. Smiselno bi bilo preveriti tudi napovedno veljavnost slovenskega prevoda, saj so v predhodnih raziskavah ugotovili, da Presejalni vprašalnik Keele STarT Back za paciente z bolečino v hrbtu in druga podobna orodja podcenjujejo tveganje za pojav kroničnosti.

Secondary title: Reproducibility and content validity of Slovene translation of the Keele STarT Back screening tool for patients with back pain
Secondary abstract: Introduction: Chronic pain is a significant social and economic burden, affecting over 630 million people worldwide. The prevalence of chronic low back pain has increased almost 3-fold from 1992 to 2006. There are Slovenian translations of two different disability measures for low back pain patients and no screening tool evaluating level of risk for chronic pain until now. Purpose: Primary purpose of the master thesis was to translate Keele STarT Back Screening Tool to Slovene language and to investigate its reproducibility as well as floor and ceiling effects. Secondary purpose was to establish recommendation for patients with low back pain including short information about low back pain and instructions for self-help. Methods: Keele STarT Back Screening Tool was formally translated into Slovene language following a multistep approach for forward and backward translation. Reproducibility was assessed using the specific agreement. Content validity was analyzed using floor and ceiling effects. Based on literature review we established short information about low back pain and instructions for self-help. Results: In total 42 patients were included. Based on Keele STarT Back Screening Tool 42,9 % were categorized in the »low risk« subgroup, 33,3 % »medium risk« and 23,8 % »high risk«. The reproducibility had the specific agreement of 91,4 % for »low risk«, 85,7 % for »medium rist« and 95,2 % for »high risk« subgroup. For the content validity, we found no floor and ceiling effects were present. Discussion and conclusion: Keele STarT Back Screening Tool was successfully translated into Slovene language. The test has excellent reproducibility and no floor and ceiling effects. Usage of STarT Back Screening tool in clinical practice will substantially improve triage and treatment of patients with low back pain and reduce work absenteeism due to low back pain. With the proper triage patients will receive suitable treatment for their condition sooner. Further studies are needed to assess predictive validity. Authors have found that Keele STarT Back and similar tools underestimate risk level for chronic pain.
