Abstract

Sedacija je del celostne obravnave pacientov v enoti intenzivne terapije, saj pri pacientu omili negativne izkušnje. Ocena nivoja sedacije pri kritično bolnih še vedno predstavlja velik izziv, saj so potrebe po sedativih različne in individualne, točkovne lestvice za ocenjevanje pa imajo določene omejitve. Na področju zdravstvene nege v EIT bi bilo potrebno ugotoviti, kateri je najbolj objektiven način ocenjevanja sedacije.V diplomskem delu smo uporabili metodo pregleda, analize in sinteze domače in tuje strokovne literature. Pregled literature smo izvedli v naslednjih podatkovnih bazah: PubMed, CINAHL, Cochrane Library, ScienceDirect. Iskalno strategijo smo prikazali v PRISMA diagramu.Sedacijske lestvice so slabo občutljive na manjše spremembe v stopnji sedacije in so le-te prej opažene z objektivnimi metodami. Nevrološki monitorji kot so BIS in spektralna entropija imajo zaenkrat še omejeno sposobnost za rutinsko uporabo v enotah intenzivne terapije, a bi lahko zanesljivo in neprekinjeno merjenje sedacije izboljšalo klinično odločanje.Zlatega standarda za ocenjevanje globine sedacije še ni – lestvice so uporabne, vendar večkrat subjektivne in z njihovo uporabo motimo udobje pacienta. Menimo, da je prihodnost ocenjevanja globine sedacije v novih tehnoloških rešitvah.

Secondary title: Assessment of sedation levels in patients in intensive care unit
Secondary abstract: Sedation is part of the integrated treatment of patients in the intensive care unit, as it diminishes negative experiences in patients. Estimation of sedation levels in critically ill still presents a great challenge, as the needs of sedatives are different and individual while scoring scales have certain limitations. In the field of nursing care in the ICU it would be necessary to determine which is the best and most objective way of assessing the sedation.In the thesis, we used the method of examination, analysis and synthesis of domestic and foreign professional literature. The literature review was carried out in the following databases: PubMed, CINAHL, Cochrane Library, ScienceDirect. The search strategy was shown in the PRISMA diagram.: Sedation scales are poorly sensitive to minor changes in the level of sedation. These are more likely to be observed using objective methods. Neurological monitors such as BIS and spectral entropy still have limited capacity for routine use in intensive care units, but reliable and continuous sedation measurement could improve clinical decision making.There is no gold standard to assess the depth of sedation – sedation scales are useful, but often subjective and impeding patients comfort with their use. We believe that the future of assessing the depth of sedation is in new technological solutions.
