Abstract

Paliativna oskrba je namenjena obravnavi pacientov z neozdravljivo napredovalo boleznijo s ciljem izboljšati in vzdrževati ustrezno kakovost življenja umirajočega. Namen diplomskega dela je bil s pregledom literature predstaviti značilnosti paliativne oskrbe starostnikov z demenco v domovih za ostarele. Raziskovalne metode: opravili smo sistematični pregled literature. Vire smo pridobili iz podatkovnih baz CINAHL in Medline. Iskanje smo izvedli s kombinacijami ključnih besed: paliativna oskrba, demenca, zdravstvena nega, medicinska sestra, dom starostnikov v angleškem jeziku in Boolovima operatorjema AND oz. OR. V analizo smo vključili najnovejše vire, in sicer objavljene med letoma 2009 in 2019 ter iz 1121 zadetkov iskanja z vključitvenimi kriteriji izluščili 13 virov, ki so vključeni v končni pregled. Rezultati: v okviru vsebinske analize smo razvili tri teme: paliativna oskrba starostnikov z demenco v domovih za starostnike; obstoječe intervencije paliativne oskrbe starostnikov z demenco v domovih za starostnike; pomembni elementi paliativne oskrbe pri starostniku z demenco. Diskusija in zaključek: paliativna oskrba z demenco je še vedno slabo raziskano področje. Za nedostopnost paliativne oskrbe v domovih za ostarele je krivo premalo izobraženo zdravstveno osebje s področja paliativne zdravstvene nege. Posledica tega pa je neprepoznavanje ali prepozno prepoznavanje potrebe po vključitvi paliativne oskrbe pri starostniku. Obstaja premalo specializiranih timov, pomanjkanje strokovnih smernic za II izvajanje ter premalo ustrezne dokumentacije za vodenje bolnikov v paliativni oskrbi. Potrebne bi bile dodatne raziskave o učinkovitosti celostne paliativne oskrbe, in sicer o izidu ukrepov in o vplivu na skrbnike in družine.

Secondary abstract: palliative care is designed to treat patients with an incurable disease progression with the goal of improving and maintaining an adequate quality of life for the dying person. The purpose of the diploma thesis was to present the characteristics of palliative care of elderly people with dementia in homes through a literature review. Methods: a systematic review of literature has been carried out. Resources were acquired by CINAHL and Medline databases. The search was performed using keyword combinations: palliative care, dementia, nursing, nurse, home for elderly in English language with Bool’s operators AND and OR. The analysis included sources published between 2009 and 2019, and out of 1121 searches, 13 sources were included with content criteria in the final review. Results: within the framework of content analysis, we highlighted three topics, namely characteristics of palliative care of elderly people with dementia in old people's homes; existing interventions for palliative care of elderly people with dementia in homes; important elements of palliative care in the elderly with dementia. Discussion and conclusion: palliative care for dementia is still a poorly researched area. The lack of knowledge of palliative care in nursing homes is attributable to undereducated nursing staff in palliative care. As a result, the need to include palliative care in the elderly is not recognized or recognized late. There is a lack of specialized teams, a lack of professional implementation guidelines, and insufficient documentation to guide IV patients in palliative care. Further research on the effectiveness of integrated palliative care would be needed, namely the outcome of the measures and the impact on caregivers and families.
