Abstract

Uvod: Postopki mobilizacije živčevja, ki povzročijo povečano napetost ali drsenje živčevja, se pogosto uporabljajo za določanje diagnoze in zdravljenje bolečine v vratu nevrološkega izvora. Namen prispevka je predstaviti učinke mobilizacije živčevja pri pacientih z bolečino v vratu nevrološkega izvora. Metode: Literatura je bila pregledana po elektronskih podatkovnih zbirkah CINAHL in PubMed. Rezultati: Analizirali smo enajst randomiziranih kontroliranih poskusov. Rezultati so pokazali, da pri pacientih z bolečino v vratu nevrološkega izvora različne tehnike mobilizacije živčevja, ki povzročijo povečano napetost ali drsenje živčevja z uporabo drugih fizioterapevtskih postopkov ali brez njih, statistično značilno (od p < 0,01 do p < 0,05) zmanjšajo bolečino, povečajo stopnjo funkcionalnosti in sklepno gibljivost. Največkrat je bila uporabljena tehnika drsenja živčevja. Dolgoročni učinek mobilizacije živčevja ni znan. Zaključki: Ugotovili smo veliko raznolikost pri uporabi tehnik in drugih merjenih spremenljivk. V prihodnjih raziskavah bi bilo treba natančno opredeliti tehniko, trajanje in intenzivnost ter meriti dolgoročni učinek. To bi izboljšalo metodološko kakovost raziskav in dokazljivost učinkovitosti mobilizacije živčevja pri pacientih z bolečino v vratu nevrološkega izvora.

Secondary title: Effects of neural mobilization at patients with neurologic neck pain
Secondary abstract: Introduction: Neural mobilization techniques that promote either neural tensioning or sliding are widely used to assess and treat neck pain with a neuropathic component. The purpose was to review literature that aimed to determine the effectiveness of neural mobilization in patient with radicular neck pain. Methods: Database search included CINAHL and PubMed with the following search terms: neural mobilization [Title/Abstract] AND neck pain, neurodynamics [Title/Abstract] AND cervicobrachial pain, cervical radiculopathy. The search included the articles from years 2000 to 2018. Results: Eleven randomized controlled trials were selected. For most of the clinical outcomes, neural mobilization techniques which increase tension or sliding of the nerves, showed to be statistically significantly (p = < 0.01 to p = < 0.05) effective for improvement in pain, disability and range of motion for the groups receiving neural mobilisation in comparison with other treatment. Sliding techniques seemed to be more effective. No long term effect of neural mobilization is known. Conclusion: The neural mobilization techniques and other parameters varied greatly between studies. Future studies should report detailed techniques, duration, intensity and long term effect in order to improve methodological quality of studies and support the found evidence from this review.
