Abstract

Uvod: Funkcijska ocena hoje za paciente z okvaro hrbtenjače (angl. spinal cord injury functional ambulation inventory – SCI-FAI) je standardizirano merilno orodje za oceno kinematike, pripomočkov in premičnosti. Namen raziskave je bil oceniti zanesljivost posameznega preiskovalca, zanesljivost med preiskovalci, sočasno veljavnost in učinek stropa slovenskega prevoda SCI-FAI. Metode: V raziskavi je sodelovalo 30 pacientov z nepopolno okvaro hrbtenjače. Za obe obliki zanesljivosti smo izračunali intraklasne korelacijske koeficiente (ICC). Za oceno veljavnosti smo izračunali Pearsonove (r) ali Spearmanove (ro) korelacijske koeficiente. Učinek stropa smo izrazili v odstotkih preiskovancev, ki so dosegli največje možno število točk. Rezultati: Ugotovili smo visoko zanesljivost posameznega preiskovalca (ICC = 0,82−0,86) in nizko (ICC = 0,46) do srednjo (ICC = 0,61) zanesljivost med preiskovalci. Sočasna veljavnost s testi hoje je bila dobra (ro = 0,61−0,62) do zelo dobra (r = 0,98−0,99; ro = 0,79−0,92). Največje število točk pri oceni pripomočkov je doseglo 47 % preiskovancev, pri oceni premičnosti pa 73 %. Zaključki: Drugi avtorji so prav tako ugotovili visoko zanesljivost posameznega preiskovalca, dobro sočasno veljavnost s testi hoje in učinek stropa za posamezne dele SCI-FAI. Zaradi nizke do srednje zanesljivosti med preiskovalci priporočamo, da hojo s SCI-FAI pred in po obravnavi ocenjuje isti fizioterapevt.

Secondary abstract: Background: Spinal cord injury functional ambulation inventory (SCI-FAI) is a standardised outcome measure for assessment of kinematics, assistive devices and mobility. The purpose of our study was to evaluate intra-rater and inter-rater reliability, concurrent validity and ceiling effect of the Slovenian translation of SCI-FAI. Methods: 30 patients with incomplete spinal cord injury participated in the study. We calculated intraclass correlation coefficients (ICC) for both reliability measures. For assessing validity, we calculated Pearsons (r) and Spearmans (ro) correlation coefficients. Ceiling effect was reported as percentage of participants achieving maximal possible score. Results: We found good intra-rater reliability (ICC = 0.82–0.86) and low (ICC = 0.46) to moderate (ICC = 0.61) inter-rater reliability of the SCI-FAI. Concurrent validity with walk tests was good (ro = 0.61–0.62) to excellent (r = 0.98–0.99; ro = 0.79–0.92); 47 % of the participants achieved maximal possible score for assistive devices assessment, 73 % for mobility assessment. Conclusions: Other authors also observed good intra-rater reliability, good validity and ceiling effect of the SCI-FAI. Because of low to moderate inter-rater reliability, we recommend that gait assessment with SCI-FAI is performed by the same physiotherapist before and after treatment.
