Abstract
Uvod: Namen študije primera je bil ugotoviti učinkovitost ishemične vadbe na velikost in zmogljivost mišice quadriceps femoris po artroskopski operaciji kolenskega sklepa. Metode: V enomesečni program ishemične vadbe je bil vključen pacient, star 29 let, 15 dni po artroskopski resekciji meniskusa. Vse meritve in testiranja smo opravili dvakrat, in sicer pred terapevtskim programom in po njem. Izvedli smo meritve obsega stegna, test enonožnega počepa ter meritve navora maksimalne izometrične kontrakcije in izotonične vzdržljivosti ekstenzorjev kolena. Rezultati: Po obdobju ishemične vadbe se je večina merjenih spremenljivk operirane noge izrazito izboljšala. Obseg stegna se je povečal za 3,4 cm, navor izometrične kontrakcije ekstenzorjev kolena se je povečal za 16 %, vzdržljivost se je izboljšala za 17 %. Hkrati se je za 20 % povečala amplituda EMG-aktivnosti, 13 % mišična poraba kisika in 20 % skupna prekrvitev mišice. Sklep: Ishemična vadba proti majhnemu uporu lahko klinično pomembno izboljša velikost, jakost in vzdržljivost mišic ekstenzorjev kolena v zgodnjem obdobju po artroskopski resekciji meniskusa. Varnost in učinkovitost ishemične vadbe pri okvarah kolena je treba ovrednotiti na večjem vzorcu.

Introduction: The aim of this case study was to evaluate the effects of ischemic exercise training on quadriceps muscle size and performance after arthroscopic knee surgery. Methods: The patient, a 29-year old male, attended 4- weeks’ program of ischemic exercise 15 days after arthroscopic partial meniscectomy. The thigh circumference, single leg squat test, maximal volitional isometric contraction torque and isotonic endurance of knee extensors were assessed before and after the training program. Results: Most of the measured variables significantly improved after the program. Thigh circumference, maximal isometric torque and isotonic endurance increased by 3.4 cm, 16% and 17%, respectively. This was paralleled with increases in EMG amplitude by 20%, muscle oxygen consumption by 13% and total muscle perfusion by 20%. Conclusion: Low-resistance ischemic exercise has clinically significant effect on the improvement of knee-extensor muscle size, strength and endurance in the early phase after arthroscopic knee meniscectomy. Safety and efficiency of ischemic exercise need to be evaluated in larger population. |
