Abstract
Uvod: Povečanje obsega gibljivosti skočnih sklepov je pogost fizioterapevtski cilj. Dorzalno fleksijo skočnega sklepa je za oceno funkcioniranja primerneje meriti v stoječem kot v razbremenjenem položaju. Namen pregleda literature je bil ugotoviti, kateri postopki merjenja dorzalne fleksije skočnega sklepa v stoječem položaju imajo najboljšo zanesljivost in bi jih lahko priporočili za klinično uporabo. Metode: Raziskave so bile iskane v podatkovni zbirki PubMed. Rezultati: V pregled je bilo vključenih 14 raziskav, v katerih so preučevali različne postopke merjenja obsega dorzalne fleksije skočnega sklepa v stoječem položaju z univerzalnim ali gravitacijskim goniometrom, mobilno aplikacijo goniometra ter centimetrskim merilnim trakom ali kotnikom pri zdravih preiskovancih in po poškodbah skočnih sklepov. Poročali so o zmerni do zelo dobri zanesljivosti posameznega preiskovalca (ICC = 0,65−0,99) in dobri do zelo dobri zanesljivosti med preiskovalci (ICC = 0,80−0,99). Zaključki: Merjenje dorzalne fleksije skočnega sklepa stoje je zanesljivo in preprosto. Kadar je dovoljeno obremenjevanje spodnjega uda, lahko nadomesti standardno metodo merjenja v ležečem položaju. Za fizioterapevtsko ocenjevanje pri pokrčenem in/ali iztegnjenem kolenu priporočamo uporabo gravitacijskega goniometra ali mobilne aplikacije goniometra, nameščenega na Ahilovo tetivo, za motivacijo pacienta pa merjenje s centimetrskim merilnim trakom.

Reliability of the weight-bearing ankle joint dorsiflexion range of motion measurement - literature review |

Background: Increase of the ankle joint range of motion is a common physiotherapy goal. Measurement of the ankle joint dorsiflexion in weight bearing is more appropriate to assess functioning than measurement in non-weight-bearing positions. The purpose of the literature review was to establish which ankle joint dorsiflexion measurement procedures are the most reliable and might be recommended for clinical use. Methods: a literature review was conducted using database PubMed. Results: 14 studies investigating different measurement procedures of the ankle joint dorsiflexion motion, using a standard goniometer, bubble/digital inclinometer, smartphone application, and tape measure in healthy or injured subjects, were included. Moderate to very good intratester (ICC = 0.65−0.99) and good to very good intertester (ICC = 0.80−0.99) reliability was reported. Conclusions: Measurement of the ankle joint dorsiflexion in weight-bearing position is reliable and might replace the standard method in non-weight-bearing position, if there is no restriction for weight-bearing on the limb. For physiotherapy assessment with the knee flexed and/or extended, we recommend the use of bubble goniometer or smartphone application, applied on Achills’ tendon, and tape measurement for patient’s motivation. |
