Abstract

Teoretična izhodišča: Terapija z negativnim tlakom je neinvazivna oblika terapije, ki se uporablja za zdravljenje akutnih ali kroničnih ran. S pomočjo posebne naprave ustvarja kontroliran negativni tlak in s tem pospešuje sekundarno celjenje, tako da ustvarja za to idealno okolje. Namen zaključnega dela je bil raziskati uporabo in učinkovitost terapije z negativnim tlakom pri obravnavi kroničnih ran. Metodologija: Uporabljen je bil pregled in analiza znanstvene ter strokovne literature v podatkovnih bazah PubMed, ScienceDirect in CINAHL ter dodatno še COBISS in Google Scholar. Iskanje je potekalo od avgusta do oktobra 2019. Identificiranih je bilo 790 zadetkov, od katerih je bilo v končno analizo na podlagi zadanih kriterijev vključenih 10 člankov, ki so bili vsebinsko analizirani. Rezultati: Terapija z negativnim tlakom je najuspešnejša pri zdravljenju diabetičnih razjed stopala. Za zdravljenje golenskih razjed ter razjed zaradi pritiska zaenkrat še ni dovolj opravljenih kakovostnih raziskav, čeprav se vseeno kažejo pozitivni učinki. V večini primerov se uporablja kot tehnika za sekundarno celjenje ran, prav tako se pogosto uporablja tudi v namene hitrejše priprave rane na nadaljnjo kirurško oskrbo. Ugodno vpliva tudi na preživetje kožnih presadkov, saj izboljša krvni obtok in jih imobilizira. Diskusija in zaključek: Glede na pregledano literaturo je pri večini kroničnih ran terapija z negativnim tlakom uspešnejši način zdravljenja kot standardne obloge. Čeprav je potrebnih še več kakovostnejših raziskav, so vseeno vidni pozitivni učinki na hitrost celjenja ran. Vedno pogosteje se uporablja tudi v slovenskih bolnišnicah.

Secondary abstract: Introduction: Negative pressure wound therapy is a non-invasive type of therapy used for treating acute and chronic wounds. With the help of a special device, it generates controlled negative pressure, which accelerates healing by secondary intention, creating an ideal environment for wound healing. The aim of diploma thesis was to examine the use and effectiveness of negative pressure wound therapy in chronic wound care. Methodology: A review and analysis of literature was conducted using databases PubMed, ScienceDirect and CINAHL, in addition to COBISS and Google Scholar. The search was conducted from August to October 2019. 790 hits were identified, of which 10 articles were included in the final analysis on the basis of the inclusion and exclusion criteria and analyzed using content analysis. Results: Negative pressure wound therapy is most successful in treating diabetic foot ulcers. For the treatment of leg and pressure ulcers, no rigorous research has been carried out so far, although there is some evidence of positive effects on wound healing. In most cases, it is used for healing wounds by secondary intention and is also often used for faster preparation of the wound bed for further surgical care. It also has a beneficial effect on the survival of skin grafts as it immobilizes them and improves blood circulation. Discussion and Conclusion: Negative pressure wound therapy is more successful in treating most chronic wounds than traditional dressings, although more rigorous evidence is needed. Positive effects on wound healing were discovered with its use, especially on the time to complete wound closure. It is also increasingly used in Slovenian hospitals.
