Abstract

Uvod: Poznavanje vzrokov za napake pri dajanju zdravil in ovir za nesporočanje prispeva k varnejši in kakovostnejši zdravstveni obravnavi, zato smo v zaključnem delu želeli raziskati napake pri dajanju zdravil v enem od socialno varstvenih zavodov. Metode: Uporabili smo kvantitativno metodologijo in deskriptivno metodo dela. Podatke smo zbrali s pomočjo anketnega vprašalnika na priložnostnem vzorcu 80 zaposlenih v zdravstveni negi v enem od socialno varstvenih zavodov v severovzhodnem delu Slovenije ter jih analizirali s pomočjo deskriptivne statistike. Rezultati: Anketiranci so med glavne vzroke za napake pri dajanju zdravil navedli neustrezno kadrovsko zasedbo (M = 3,91; SD = 1,50), motnje s strani stanovalcev (M = 3,86; SD = 1,59) in prekinitve zaradi drugega dela (M = 3,80; SD = 1,54). 97,5 % anketirancev je ocenila, da pozna protokol za prijavo napake, vendar se vse napake ne sporočajo zaradi osredotočanja na posameznika namesto na iskanje vzrokov v sistemu (M = 3,01; SD = 1,60), negativnega odziva stanovalcev in svojcev na napako (M = 2,85; SD = 1,39) ter strahu pred posledicami sporočanja (M = 2,78; SD = 1,43). 72,5 % anketirancev je ocenilo, da se sporoči več kot 60 % vseh napak na oddelku. Razprava in sklep: Z identifikacijo individualnih in sistemskih vzrokov za napake in ovir za sporočanje lahko v naslednjem koraku iščemo rešitve in izboljšujemo sistem dela. Z vzpostavitvijo neobtožujočega okolja lahko izboljšamo sporočanje napak.

Secondary title: Safety in medication administration in a long term care
Secondary abstract: Introduction: Knowing the causes of medication administration errors and barriers of reporting contributes to safer and better-quality of patient care, therefore the aim of diploma thesis was to research administration medication errors in a long term care. Methods: Quantitative methodology and descriptive research method were used. Data were collected using questionnaire on a convenience sample of 80 nursing staff in a long term care in the North-eastern Slovenia and analysed data with descriptive statistics. Results: Amongst the main causes of medication administration errors, the respondents stated inadequate staffing (M = 3,91; SD = 1,50), disruptions (M = 3,86; SD = 1,59) and interruptions (M = 3,80; SD = 1,54). 97,5% of the respondents assessed they know the protocol for reporting errors, however do not report every error because of focusing on the individual instead of searching for causes in the system (M = 3,01; SD = 1,60), negative response of the residents and their relatives to the error (M = 2,85; SD = 1,39) and fear of the consequences for reporting it (M = 2,78; SD = 1,43). 72,5% assessed that more than 60 % of all error are reported. Discussion and conclusion: By identifying individual and systemic causes for errors and barriers of reporting we can search for solutions and improve the work system by taking the next step. We can increase reporting medication errors by establishing non-blaming environment.
