Abstract
Uvod: Pri obravnavi kritično bolnih otrok v nujni medicinski pomoči je ena najpomembnejših intervencij vzpostavitev žilnega dostopa. Ker je vstavitev intravenske kanile velikokrat zelo otežena ali celo nemogoča, morajo zdravstveni delavci imeti potrebno znanje in izkušnje, da v takih primerih uporabijo intraosalni pristop. Namen diplomskega dela je raziskati znanje zaposlenih v nujni medicinski pomoči glede uporabe intraosalnega pristopa pri otrocih in v kolikšni meri ga uporabljajo.
Metode: Za pripravo teoretičnih izhodišč smo pregledali domačo in tujo strokovno literaturo. Pri empiričnem delu smo uporabili kvantitativno metodologijo raziskovanja in deduktivni pristop. Instrument raziskovanja je bil anketni vprašalnik zaprtega tipa. Anketirali smo 33 zaposlenih s področja zdravstvene nege, ki delujejo v predbolnišnični nujni medicinski pomoči.
Rezultati: Ugotovili smo, da zaposleni v predbolnišnični nujni medicinski pomoči uporabe intraosalnega pristopa pri otrocih ne poznajo dovolj dobro. Tudi sami so ocenili, da imajo premalo znanja in praktičnih veščin. Največje vrzeli v znanju smo ugotovili pri poznavanju nekaterih absolutnih kontraindikacij, zapletov in možnosti uporabe. Le dobra tretjina vključenih je pravilno izbralo mesto nastavitve. Večina jih večkrat poizkuša z vstavljanjem intravenske kanile tudi pri življenjsko ogroženih otrocih, kot pa se odloča za zgodnjo uporabo intraosalnega pristopa.
Razprava in sklep: Zaposleni v nujni medicinski pomoči bi morali imeti ustrezno znanje in veščine glede intraosalnega pristopa, saj se med delom srečujejo tudi s primeri otežene ali nemogoče vstavitve intravenske kanile pri življenjsko ogroženih otrocih. Tu je njihovo znanje lahko odločilno, zato bi ga morali redno nadgrajevati.

Use of intraosseous access in pediatric emergency care |

Introduction: In pediatric emergency care vascular access is one of the most important interventions. However, establishing a peripheral intravenous catheter can be difficult or even impossible. Therefore, healthcare workers must have clinical expertise to use intraosseous access. The aim is to find out to what extent of healthcare workers in emergency care use intraosseous access in children and what knowledge they have about it.
Methods: The theoretical basis is summarized from scientific literature and articles. For empirical research we use a quantitative research method with deductive approach. The research was carried out with the help of a questionnaire among 33 paramedics in prehospital emergency care.
Results: The knowledge regarding intraosseous access in children among paramedics in prehospital emergency care is poor. The majority of participants in the survey believe they do not have enough theoretical knowledge and skills. Biggest knowledge gaps were found in knowing absolute contraindications, complications and usage options. Furthermore, only 37.5 % of participants choose the right site for intraosseous access in children. Regarding the results of the survey paramedics attempt peripheral intravenous access multiple times, even in critically ill children, rather than use intraosseous access.
Discussion and conclusion: Healthcare professionals in emergency care should have appropriate knowledge and skills regarding the intraosseous access, as they also encounter cases of difficult or impossible insertion of an intravenous cannula in critically ill children. In situations like these their knowledge can be crucial and therefore should be regularly upgraded. |
