Abstract

Uvod: Telesna krhkost je posledica staranja in bolezni, pri kateri starejši odrasli potrebujejo pomoč pri osnovnih življenjskih aktivnostih. H krhkosti vodita zmanjšana mišična sila in moč, za katero je značilna izguba telesne mase, zmanjšana zmožnost telesne vadbe, upočasnjena hoja, izčrpanost in slabotnost. Vprašalnik FiND je inštrument za odkrivanje krhkosti pri starejši populaciji prebivalstva. Namen: Namen magistrskega dela je prevesti vprašalnik FiND in ga prilagoditi slovenskemu govornemu področju in kulturnemu okolju ter preveriti njegovo notranjo skladnost, zanesljivost, veljavnost, občutljivost in specifičnost na vzorcu starejših odraslih ljudi, samostojno živečih v domačem okolju. Metode dela: Presejalni vprašalnik FiND smo po protokolu, ki ga priporočajo Beaton in sodelavci (2000) prevedli iz angleškega jezika in nazaj. Notranjo skladnost vprašalnika smo preverjali s Cronbach koeficient ?, njegovo zanesljivost smo s ponovljenim merjenjem preverjali z Intraklasnim koeficientom korelacije. Povezanost med FiND-S vprašalnikom in indeksom krhkosti smo preverjali s Spearmanovim koeficientom korelacije. Na koncu smo z ROC analizo preverili še občutljivost in specifičnost vprašalnika. Rezultati: V raziskavo je bilo vključenih 30 starejših odraslih. Z vprašalnikom FiND-S smo ugotovili 25 krepkih in pet krhkih preiskovancev. Z indeksom krhkosti glede na rezultate fenotipa smo ugotovili, da je bilo 6 preiskovancev krhkih in 24 krepkih. Cronbach koeficient ? je znašal 0,831 (95% spodnja meja zaupanja = 0,781). Pri prvem in drugem testiranju je znašala mediana za vprašalnik FiND-S 0,0 (95 % interval, zaupanja = 0,0-0,0), razlika pa ni bila statistično pomembna. Dobljeni ICC je znašal 0,868 (0,720-0,937). Povezanost med FiND-S in indeksom krhkosti je bila visoka (? = 0,873 (0,749 - 0,938)) in statistično pomembna (p < 0,0001). Pri ROC analizi je bila površina pod krivuljo več kot 0,9 in statistično pomembna. Pri kriteriju, da posameznik doseže pri vprašalniku FiND-S več kot nič točk je znašala občutljivost 83,3 %, specifičnost pa 100 %. Razprava in zaključek: Glede na rezultate pilotne raziskave lahko zaključimo, da je prevod vprašalnika FiND ustrezen v slovenskem jezikovnem in kulturnem okolju. Priložnost za fizioterapevtsko stroko vidimo predvsem v uporabi vprašalnika na primarnem nivoju zdravstva.

Secondary title: Slovenian translation, reliability and validity of the Frail non-disabled questionnaire
Secondary abstract: Introduction: Frailty is a common geriatric syndrome including the reduction in functional status which necessitates the provision of support in the performance of basic daily activities. Frailty, highly prevalent in old age, is caused by diminished muscle mass and muscle strength characterised by unintended body mass loss, decreased physical activity, slowed walking speed, exhaustion and weakness. The Frail Non-Disabled questionnaire – FiND is one of the instruments used in frail non-disabled individuals for timely recognition of frailty. Purpose: The purpose of the master thesis was to translate the FiND questionnaire and modify it to accord the Slovene linguistic properties and culture and verify its internal consistency, reliability, validity, sensitivity and specificity in the sample of older adults living independently in their home environment. Methods: The screening FiND questionnaire was translated from English into Slovene and back-translated observing the protocol recommended by Beaton and colleagues (2000). The inner consistency was tested with the Cronbach coefficient α and its reliability with the repeated measurement with the intraclass correlation coefficient. The correlation between the FiND-S questionnaire and the frailty index was additionally tested with the Spearman's correlation coefficient. In the final stage, the sensibility and specificity of the questionnaire were tested with the ROC analysis. Results: The research sample consisted of 30 older adults. Using the FiND-S questionnaire, it was established that 25 of the participants were robust and five of them were frail. According to the frailty phenotype and the frailty index, 24 participants were robust and the remaining 6 were frail. The inner consistency of the questionnaire was acceptable since the Cronbach coefficient  amounted to 0.831 (the lowest confidence level = 0.781, i.e. 95 %). In the first and second testing, the median for the FiND-S questionnaire was 0.0 (confidence interval = 0.0-0.0; 95 %), but the difference was not statistically significant. The obtained interclass correlation coefficient was 0.868 (0.720-0.937), which means that the questionnaire has a high reliability. The correlation between the FiND-S questionnaire and the frailty index was high ( = 0.873 (0.749 – 0.938)) and statistically significant (p < 0.0001), which confirms the validity of the FiND-S questionnaire. In ROC analysis, the area under the curve was over 0.9 and is statistically significant. Given that an individual's score in the FiND-S questionnaire is above the threshold t=0, the sensitivity was 83.3% and the specificity 100%. Discussion and conclusion: Results of the pilot study indicate that the translation of the FiND-S questionnaire is satisfactory and suitable for use in the Slovene linguistic and cultural environment. The questionnaire enables self-assessment and recognition of frailty symptoms. The physiotherapist may find the questionnaire of great advantage in everyday practice on the primary level of healthcare.
