Abstract
Bolečina v križu je eden največjih globalnih problemov, s katero naj bi se v svojem življenju srečale kar tri četrtine ljudi. Gre za kompleksno težavo, ki je posledica veliko soodvisnih biopshihosocialnih dejavnikov. Veliko pacientov trpi za kronično bolečino v križu, ki predstavlja velik zdravstveni in ekonomski problem. Fizioterapevtska obravnava bolečine v križu zajema različne metode, katerih del je tudi vadba. V zadnjih letih so priljubljene vaje za stabilizacijo trupa, zato smo z diplomsko nalogo želeli preveriti, ali so dejansko učinkovitejše od standardne vadbe, in podati predloge za vadbo pri pacientih s kronično bolečino v križu. Pregled literature je pokazal, da sta obe obliki vadbe srednje- in dolgoročno enako učinkoviti, vendar lahko zagovarjamo, da so vaje za stabilizacijo trupa učinkovitejše pri zmanjšanju bolečine in povečanju zmanjšanih zmožnosti samo kratkoročno. Prav zato v prvi fazi rehabilitacije priporočamo uporabo vaj za stabilizacijo trupa, da paciente naučimo aktivacije globokih mišic trupa. Izkazalo se je, da le malo študij paciente spremlja v daljšem časovnem obdobju, kar je pri kronični bolečini izjemnega pomena. Opazili smo veliko pomanjkanje dolgoročnih programov vadbe, saj se zdi, da so osnovne vaje za stabilizacijo trupa premalo intenzivne, da bi pacienti samo z njihovo uporabo ohranjali dolgotrajno stabilizacijo. Ker želimo, da stabilizacija trupa postane avtomatizirana, je ključno, da v drugem koraku rehabilitacije združimo vaje za stabilizacijo trupa in standardne vaje. Osnove stabilizacije uporabimo pri funkcionalnih vzorcih, kot sta hoja in počepanje, pa tudi standardnih vajah z lastno težo, proti uporu, povečanju mišične moči in drugih oblikah vadbe. Menimo, da bi morala stroka postaviti jasno definicijo obeh oblik vadbe, saj je ločnica med njima pogosto nejasna, nato pa pripraviti program varnih in ustreznih vaj glede na njihovo težavnost. Ker je bolečina v križu tako kompleksna, moramo vadbo vedno individualizirati ter upoštevati poklic pacienta in njegovo psihično stanje.

Effectiveness of core stabilization exercises in patients with chronic low back pain |

Low back pain is one of the most serious global problems faced by as many as three quarters of people during their lifetime. It is a complex problem that results from several interdependent bio-psycho-social factors. Many patients suffer from chronic low back pain, which presents a major health and economic difficulty. Physiotherapy treatment of low back pain comprises various methods, including exercise. Due to recent popularity of core stabilization exercises, our graduation thesis researches the effectiveness of core stabilization programmes in comparison to standard exercises, and consequently advises on the exercise plan for patients with chronic low back pain. The literature review suggests that both forms of exercise are equally effective in the intermediate and long term, but we can argue that core stabilization exercises are more effective in reducing pain and disabilities in the short term. Therefore, the first phase of rehabilitation should focus on core stabilization exercise aimed at teaching patients the activation of the deep trunk muscles. Unfortunately, only a handful of studies follow patients over a long period of time, which is extremely important in chronic pain. The lack of long-term exercise programs is problematic, as basic core stabilization exercises are not sufficiently intensive to maintain long-term stabilization. The aim of these programmes is automatic activation of stabilizing muscles; hence it is crucial to combine core stabilization exercises and standard exercises in the second phase of rehabilitation. The basics of stabilization should be used in functional patterns such as walking and squatting, as well as in body weight, resistance, strength, and other standard exercises. We believe that the profession should provide a clear definition of both forms of exercise, as the distinction between them is often blurred. Moreover, a program of safe and appropriate exercises should be prepared according to their difficulty. Considering the complexity of low back pain, exercise programmes should be individualized in view of the patient's profession and their mental state. |
