Abstract

Uvod: Telerehabilitacija predstavlja za fizioterapijo alternativen pristop, ki nudi pacientom po možganski kapi postopke za nadaljevanje rehabilitacije na domu. Med pogosteje uporabljene oblike telerehabilitacije uvrščamo uporabo telekomunikacijskih sistemov in vadbo z uporabo navidezne resničnosti. Namen: Namen diplomskega dela je bil pregledati ugotovitve raziskav o učinkovitosti vadbe za izboljšanje ravnotežja in funkcijskih sposobnosti pri pacientih po možganski kapi s telerehabilitacijo v primerjavi z vadbo z neposrednim stikom. Metode dela: Pri iskanju literature smo uporabili podatkovne zbirke PubMed, CINAHL, PEDro ter spletno knjižnico Cochrane Library. Vključili smo raziskave, v katerih so avtorji primerjali učinkovitost vadbe za ravnotežje in krepitev funkcijskih sposobnosti s telerehabilitacijo v primerjavi z vadbo z neposrednim stikom. Rezultati: V pregled smo vključili 8 randomiziranih kontroliranih poskusov, objavljenih med letoma 2012 in 2020. V petih raziskavah so avtorji preučevali učinkovitost telerehabilitacije z uporabo navidezne resničnosti, v treh pa z uporabo telekomunikacijskih sistemov. V vseh raziskavah je prišlo pri obeh skupinah do napredka, vendar večkrat brez statistično pomembnih razlik med skupinama. Med raziskavami, v katerih so uporabili telekomunikacijske sisteme, so v dveh (od treh) raziskav poročali o izboljšanju funkcijskih sposobnosti, ravnotežja in kakovosti življenja v korist eksperimentalne skupine. Med raziskavami, v katerih so uporabili vadbo z navidezno resničnostjo, pa so v eni raziskavi (od petih), poročali o izboljšanju ravnotežja in večji samostojnosti pri opravljanju nalog, v korist eksperimentalne skupine. Zaključek: Kaže, da je pri pacientih po možganski kapi telerehabilitacija enako ali bolj učinkovita za izboljšanje ravnotežja kot vadba z neposrednim stikom. Potrebne so dodatne raziskave z večjim številom preiskovancev in enotnejšimi merilnimi orodji.

Secondary abstract: Introduction: Telerehabilitation is an alternative approach to physiotherapy that offers post-stroke patients procedures to continue their rehabilitation at home. Common forms of telerehabilitation include the use of telecommunication systems and virtual reality training. Purpose: The aim of this diploma work was to review research findings on the effectiveness of balance and functional training in post-stroke patients with telerehabilitation compared to in-person training. Methods: PubMed, CINAHL, PEDro and the Cochrane Library were used for the literature search. The studies included in our review focused on the effectiveness of balance and functional training in stroke patients with telerehabilitation compared to in-person training. Results: We included 8 randomised controlled trials published between 2012 and 2020. In five studies, the authors focused on the effectiveness of telerehabilitation using virtual reality, and in three studies using telecommunication systems. In all studies, both groups made progress, but often without statistically significant differences between groups. Among the studies using telecommunication systems, two (out of three) reported improvements in functional status, balance and quality of life in favour of the experimental group. Among the studies that used virtual reality training, one study (out of five) reported improvements in balance and greater independence in task performance, in favour of the experimental group. Conclusion: Telerehabilitation has been shown to be as or more effective at improving balance than in person rehabilitation in stroke patients. Further research is needed with larger numbers of subjects and more uniform measurement tools.
