Abstract

Paliativna oskrba pomeni aktivno in celostno oskrbo, nego in spremljanje bolnikov od trenutka, ko se njihova bolezen ne odziva več na kurativno zdravljenje. Paliativna oskrba izboljša kakovost življenja bolnika in njegovih svojcev, ki se soočajo s težavami ob neozdravljivi bolezni ob tem pa ne pozablja na holističen pristop. Paliativna oskrba je v svetu že dolgoletna praksa, v slovenskem prostoru pa je manj znana oblika pomoči težko bolnim in umirajočim. Za kakovostno obravnavo težko bolnega in umirajočega pacienta se pojavi potreba po timskem sodelovanju. Potreben je paliativni tim strokovnjakov v katerega so vključeni zdravnik, medicinska sestra/zdravstvenik, klinični psiholog ,socialni delavec, fizioterapevt, delovni terapevt, dietetik, verski predstavnik in prostovoljci. V diplomski nalogi je predstavljena raziskava o timskem pristopu v negovanju težko bolnih pacientov na domu. Anketa je bila izvedena v letu 2010, v njej pa je sodelovalo 15 patronažnih medicinskih sester iz OE patronažno varstvo zdravstvenega doma dr. Adolfa Drolca Maribor, ter patronažne medicinske sestre OE patronažno varstvo zdravstvenega doma Ptuj. V anketi je sodelovalo tudi 30 svojcev, negovalcev ki skrbijo za težko bolne paciente v domačem okolju. Analiza raziskave je pokazala da si patronažne medicinske sestre želijo boljšega sodelovanja z osebnim zdravnikom pacienta, prav tako je analiza raziskave pokazala da so patronažne medicinske sestre najdejavnejše članice negovalnega, zdravstvenega tima ki prihajajo v domače okolje. Analiza raziskave je tudi pokazala da si svojci, negovalci želijo pomoči in aktivnejšega sodelovanja z osebnim zdravnikom. Tukaj je izpostavljen bistven segment paliativne oskrbe, in to je timsko sodelovanje. Brez timskega sodelovanja ne moremo ponuditi pacientom kakovostne paliativne oskrbe.

Secondary abstract: Palliative care means active and holistic approach, care and monitoring patients from moment when their diseas is not responding on curative medicine. Palliative care improves quality of life for the patient and his relatives when they are facing with problems caused by incurable diseas. For quality approach in palliative care team approach is needed. Palliative team is needed and this team involves doctor, nurse, clinical psychologist,social worker, physiotherapist, occupational therapist,dietetian, Religious Representative and volunteers. This diploma work presents a survey about team approach in nursing heavilly ill patient in home care. The survey was perfomed in year 2010 and involved 15 community nurses from health center Maribor and health center Ptuj. Also 30 relatives, caregivers participated in survey. Analysis of survey showed that community nurses need better cooperation with personal doctor of the patient and analysis also showed that community nurses are most active members of nursing and medical team wich comes into home enviorment. Analysis of the survey also showed that relatives, caregivers wish for better cooperation with personal doctor of the patient. Here is exposed a significant segment of palliative care, and this is team work. Without team work we can not offer high-quality palliative care for patients.
