Abstract
Ustrezna in uspešna obravnava kronične bolečine je primarnega pomena, saj gre za dolgotrajno bolečino, ki traja dlje kot 3–6 mesecev in pacientu uničuje fizično, psihično, socialno in ekonomsko življenje. Kronična bolečina opazno zmanjšuje posameznikovo kakovost življenja. V diplomskem delu smo se osredotočili na ustrezne psihološke vidike v fizioterapevtski obravnavi, v raziskovalnem delu naloge pa izvedli sistematični pregled tuje in slovenske strokovne literature ter skušali ugotoviti, ali ima odnos terapevt-pacient pomembno vlogo pri doseganju zastavljenih ciljev za uspešno fizioterapevtsko obravnavo ter kakšna je kakovost pacientovega življenja s kronično bolečino. Ugotovili smo, da je fizioterapija sicer ena izmed najpomembnejših obravnav kronične bolečine, vendar bi bilo priporočljivo, da bi fizioterapevt imel tudi splošna znanja o motnjah osebnosti in vedenja, stresa ter motivacije. Študije so kot največji problem ugotovile, da fizioterapevti poročajo o omejenem znanju, pomanjkanju časa ter minimalnem usposabljanju psiholoških strategij. Predlagajo, da bi se osnove vključile v študij fizioterapije. Pacienta s kronično bolečino bi bilo treba obravnavati celostno, na multidisciplinarni način, kar pomeni, da bi moral imeti fizioterapevt poleg specialnih znanj tudi znanja s področja psihologije, komunikologije in sociologije. S pomočjo teh veščin, bi se mu lahko približal, vzpostavil močnejše zaupanje in sodelovanje. Fizioterapija pri obravnavi kronične bolečine je ena izmed učinkovitih terapij, pri čemer odnos med fizioterapevtom in pacientom predstavlja pomemben člen za uspešnost fizioterapevtske obravnave. Kot najbolj učinkovit pristop zdravljenja se je izkazal multidisciplinarni način, ki poleg fizioterapije vključuje tudi psihološko obravnavo ter pacienta obravnava individualno. Kronična bolečina predstavlja poleg resnih posledic za pacientovo kakovost življenja tudi škodljiv učinek na njegovo socialno in družinsko okolje. Prav tako le-ta predstavlja tudi pomembne posledice za paciente in njegove svojce, kar povzroča poslabšanje kakovosti življenja pacientov in bližnjih.

Psychological aspects of managing chronic pain with physiotherapy |

Proper, successful treatment of chronic pain is of primary importance, since we are speaking about prolonged pain, which lasts longer than 3-6 months and ruins the physical, mental, social and economic life of the patient. Chronic pain considerably decreases the individuals’ quality of life. In the graduation thesis, we focused on relevant psychological aspects of physiotherapeutic treatment. In the research part of the thesis, we carried out a systematic review of foreign and domestic professional literature and tried to determine the quality of life of a patient with chronic pain and if the therapist-patient relationship plays an important role in the realisation of goals for successful physiotherapeutic treatment. We found that physiotherapy is one of the most important treatments of chronic pain, however, it is advisable that the physiotherapist also possesses knowledge about personality and behavioural disorders, stress and motivation. According to the studies, the most prominent issue is that physiotherapists report gaps in knowledge, lack of time and minimal training in the field of psychological strategies. It is proposed that the basic training in this regard be included in the studies of physical therapy. A patient with chronic pain should be treated in a holistic, multidisciplinary manner, which means that the physiotherapist should not only have specialized skills, but also knowledge of psychology, communication and sociology. By utilizing these skills, the physiotherapist could build a closer relationship with the patient and establish stronger trust and cooperation. When treating chronic pain, physiotherapy is one of the most effective therapies, and the relationship between the physiotherapist and the patient is an important prerequisite to ensure that the physiotherapy treatment is successful. A multidisciplinary approach that includes psychological treatment alongside physiotherapy and is tailored to the patient has proven to be the most effective treatment approach. Chronic pain not only has serious consequences for the patient's quality of life, but also has a detrimental effect on their social and family environment. It also has significant consequences for patients and their relatives, leading to a deterioration in the quality of life of both patients and their loved ones. |
