Abstract
Uvod: Nogomet je kontakten šport z visokim tveganjem za poškodbo, med katere najpogosteje sodijo zvini gležnja. Ti imajo visoko stopnjo ponovljivosti, kar je povezano s kronično nestabilnostjo gležnja. Pri fizioterapiji le-teh se pogosto uporablja manualna terapija. Nekatere tehnike, ki temeljijo na mobilizaciji talokruralnega sklepa, so dokazano učinkovite za povečanje obsega gibljivosti v gležnju ter izboljšanje artrokinematike sklepa. Namen: Namen diplomskega dela je bil raziskati takojšnje učinke enkratne manualne terapije (tehnik mobilizacije) na funkcijo gležnja pri nogometašici s kronično nestabilnim gležnjem. Metode dela: Izvedena je bila študija primera, v kateri je sodelovala aktivna nogometašica s kronično nestabilnim gležnjem. Uporabljena so bila naslednja merilna orodja in postopki: analiza gibanja središča pritiska, test dosega z nogo v osmih smereh, test poskakovanja v osmici, test poskakovanja v kvadratu, pasivna meritev dorzalne fleksije v zgornjem skočnem sklepu, merjenje dorzalne fleksije v stoječem položaju, test drsenja talusa ter ocena bolečine z vidno analogno lestvico za oceno intenzitete bolečine. Manualna terapija je obsegala dva seta različnih tehnik mobilizacije skočnega sklepa po dve minuti z vmesnim enominutnim premorom. Rezultati: Ugotovili smo, da tehniki mobilizacije skočnega sklepa vplivata na povečanje obsega giba dorzalne fleksije zgornjega skočnega sklepa in na posteriorno drsenje talusa. Mobilizacija vpliva na izboljšanje pretežno statičnega ravnotežja, pri testu na pritiskovni plošči predvsem v anteroposteriorni smeri gibanja središča pritiska. Izboljšalo se je dinamično ravnotežje, rezultat testa dosega z nogo v osmih smereh je pokazal največ izboljšav v posterolateralni smeri dosega. Manualna terapija je izboljšala tudi rezultate pri obeh funkcijskih testih, ki so povezani z gibalnimi vzorci nogometa. Razprava in zaključek: Tehnike mobilizacije skočnega sklepa lahko pomembno vplivajo na izboljšanje obsega giba dorzalne fleksije, statičnega in dinamičnega ravnotežja ter na funkcijo gležnja v povezavi z nogometno igro pri nogometašici s kronično nestabilnim gležnjem. V prihodnosti bi bilo smiselno izvesti podobno raziskavo z večjim številom preiskovank, prav tako bi bilo pomembno, da se izvede dodatna in tudi daljša intervencija.

Acute manual therapy effects on ankle function in a female soccer player with chronic ankle instability – case study |

Introduction: Soccer is a contact sport with a high risk of injury, the most common being sprains of the ankle. These have a high recurrence rate associated with chronic ankle instability. Manual therapy is often used in the rehabilitation process. Some techniques based on mobilization of the talocrural joint have been shown to be effective in increasing ankle range of motion and improving the arthrokinematics of the joint. Purpose: The purpose of this thesis is to find out the effects of manual therapy (mobilization techniques) on ankle joint function in a female soccer player with chronic ankle instability. Methods: A case study was conducted involving an active female soccer player with chronic ankle instability. The following measurement tools and procedures were used: force plate, star excursion balance test, figure of eight test, square hop test, passive measurement of dorsiflexion in the upper ankle joint, measurement of dorsiflexion in standing position, talus glide test and pain assessment with a visual analog scale to assess pain intensity. Manual therapy consisted of two 2-minute sets of two joint mobilization techniques, with a one-minute rest in between. Results: We found that ankle mobilization techniques affected the increase in range of motion of dorsiflexion and possibly posterior glide of the talus. Mobilization resulted in an improvement in static balance. Dynamic balance improved; the Star excursion balance test result showed the most improvement in the posterolateral direction. Manual therapy also improved the results in both functional tests related to movement patterns in soccer. Discussion and conclusion: Ankle mobilization techniques may have a significant effect on improving dorsiflexion range of motion, static and dynamic balance, and ankle function related to soccer play in a soccer player with a chronically unstable ankle. In the future, it would be useful to conduct a similar study with a larger number of subjects and additional and longer interventions. |
