Abstract
Skupnostna psihiatrična obravnava je sodobni pristop k zdravljenju in rehabilitaciji pacientov z duševnimi motnjami. Skupnostna psihiatrija omogoča obravnavo funkcionalno in geografsko opredeljene populacije. Za njeno uspešno izvajanje je potrebna mreža zdravstvenega in socialnega varstva, ki omogoči nepretrgano zdravljenje, urejanje bivanja, zaposlitev in socialno podporo. V tem primeru smo na severovzhodu Slovenije izvajali zdravstveno psihiatrično obravnavo za 13 pacientov s hudimi in ponavljajočimi se duševnimi motnjami. Namen raziskave je bil oceniti posledice te obravnave glede izida zdravljenja, zadovoljitve njihovih potreb in potreb njihovih svojcev, zagotovitve nepretrganosti obravnave in predvidene manjše uporabe hospitalizacij proti volji. Ocenili smo tudi dolžino in število hospitalizacij pri teh pacientih.Uporabljena je bila kvantitativna in kvalitativna metodologija. Podatke smo zbirali s pomočjo validiranih ocenjevalnih lestvic: ocenjevalna lestvica HONOS (Health of the Nation Outcome Scales), ocenjevalna lestvica CANSAS (Camberwell Assessment of Need-short version) in intervju za oceno potreb sorodnikov, ki skrbijo za paciente s shizofrenijo na podlagi vprašalnika avtorja Wancata et.al., 2006.
Sklep. Raziskava je pokazala ugodne učinke organiziranega strokovnega pristopa pri obvladovanju duševnih motenj pri izbranih pacientih. Približanje dejavnosti v pacientovo ožje življenjsko okolje zniža stopnjo hospitalizacije in stabilizira bivanje v družinskem in delovnem okolju.

Community psychiatric care for people with mental health disorders is a contemporary therapeutic and rehabilitation approach. The community psychiatric team follows mental health needs of geographically and functionally defined population. A mental health network providing continuity of treatment, supported accomodation facilities, supported employment and social support are organized in defined areas. In this reasearch a community psychiatric team was working in large area, using outreach and treatment for 13 identified patients with severe mental disorders in the northeast region of Slovenia.The purpose of this thesis is to research the consequences of this program regarding outcome for patients, meeting their needs, providing continuity of care, reducing use of restraint and human rights coercion and prevent disorders’ relapse.We used quantitative and qualitative methodology. Data were collected using validated rating scales: HONOS evaluation scale (Health of the Nation Outcome Scales), CANSAS evaluation scale (Camberwell Assessment of Need-short version) and The ‘‘Carers’ needs assessment for Schizophrenia (Wancatta et al., 2006).Conclusion. Community psychiatric approach in 13 patients in NE Slovenia improved their functioning and reduced hospital use and hospitalization length. |
