Abstract

Opis problema: Namen diplomskega dela je predstaviti preventivni obisk patronažne medicinske sestre pri starostniku na njegovem domu. Patronažna medicinska sestra vzpostavi stik s starostnikom, njegovo družino in okoljem, v katerem živi. Ugotavlja potrebe po zdravstveni negi, načrtuje intervencije zdravstvene nege, jih izvaja in vrednoti dosežene cilje. Metodologija raziskovanja: V raziskavi smo uporabili deskriptivno metodo dela. Podatke smo pridobili s pomočjo anonimnega anketnega vprašalnika, ki je sestavljen iz vprašanj zaprtega in odprtega tipa. V raziskavo je bilo vključenih sto sedem patronažnih medicinskih sester, ki delujejo na območju Celja, Lenarta, Maribora, Murske Sobote in Šmarja pri Jelšah. Rezultati raziskave: Rezultati raziskave so pokazali, da so patronažne medicinske sestre predvsem sestre z več leti delovne dobe in posledično v več izkušnjami. Prevladujejo sestre z višjo ali visoko izobrazbo, sledijo pa sestre s srednjo izobrazbo in z več leti delovne dobe. Vse pa zelo dobro poznajo svoje terensko območje. O pomembnosti dveh patronažnih obiskov pri starostniku imajo zelo deljena mnenja. Vsekakor pa se zavedajo problemov starajoče se populacije in težav, ki jih prinaša starost. Anketni vprašalnik je zajel obravnavo starostnika po štirinajstih življenjskih aktivnostih in oceno samooskrbe. Anketirane so izpostavile izredno koristnost preventivnih obiskov, pogosto pa se počutijo nemočne pri reševanju njihovih problemov. Razprava: Slabih izkušenj iz preventivnih obiskov nimajo pogosto, občasno pa doživljajo zavrnitve zaradi nepoznavanja, splošnega nezaupanja; starostniki menijo, da nimajo težav oziroma zanje ne potrebujejo pomoči. Pri koordinaciji z osebnim zdravnikom dobro sodelujejo, s socialno službo so pa pogosteje nezadovoljne. Zelo pogosto opažajo znake psihičnega in fizičnega nasilja nad starostniki in izpostavljajo težavnost reševanja teh problemov. Dokumentiranje pa predstavlja ključni element vsega njihovega dela. Predlagajo posodobljeno, poenoteno in računalniško dodelano dokumentiranje, izdelano po strokovnih smernicah in po procesni metodi dela. Sklep: Podatki potrjujejo izpostavljeno problematiko in dokazujejo nujnost izvajanja preventivnih patronažnih obiskov pri starostniku v predpisanem obsegu in na način, ki kakovostno in celostno obravnava starostnika v domačem okolju.

Secondary title: PREVENTIVE VISITS OF COMMUNITY NURSES TO ELDERLY PEOPLE
Secondary abstract: Problem: The aim of this dissertation is to present community nurses’ preventive visit of elderly people at their homes. The community nurse makes contact with the elderly persons, their families and the people living around them. She identifies the needs of medical care, plans interventions of medical care, carries them out and evaluates the achievements. Methodology: Descriptive method was used in the research. Data was acquired by anonymous questionnaire consisting of the open and closed type questions. The research included one hundred and seven community nurses, covering the area of cities Celje, Lenart, Maribor, Murska Sobota and town Šmarje pri Jelšah. Results: The results have shown that community nurses are mostly nurses with many years of service and therefore more experience. Nurses with short-cycle college or university degree are prevailing, following are nurses with secondary school education and longer period of service. All are very well acquainted with their local area of service. Their opinions on two preventive visits of elderly people differ. They are definitely aware of the problems appearing by ageing of the population and the problems of old age in general. The questionnaire included the treatment of an elderly person according to fourteen life activities and the estimation of self-sufficiency. The interviewees have emphasised great benefits of preventive visits, yet they often feel helpless when they encounter problems. Discussion: Community nurses rarely have negative experience from the preventive visits. Sometimes they are rejected due to the unfamiliarity of the people with the programme and general distrust, or the elderly believe they have no problems or do not need any help with them. The nurses work well in coordination with the family doctor, whereas, more difficulties appear in cooperation with the Social Work Centres. They notice many cases of physical and psychological violence towards the elderly. Record keeping, on the other hand, presents the key element of their work. They suggest updated, uniform and computerized record keeping prepared according to professional guidelines and process working method. Conclusion: The data confirm the exposed problems and prove the necessity of preventive visits of community nurses to elderly people within the due extent and with a quality and integrated approach towards elderly people in their home environment.
