Abstract

Uvod in namen: Za tendinopatijo patelarnega ligamenta (TPL) je značilna bolečina v sprednjem delu kolenskega sklepa. Najpogostejši vzrok je prekomerno obremenjevanje patelarnega ligamenta v obliki skokov. Prva izbira je konzervativno zdravljenje, pri čemer pa učinkovitost številnih fizioterapevtskih metod in tehnik ni povsem znana. Metode: Diplomska naloga temelji na sistematičnem pregledu literature, pridobljene s pomočjo elektronskih baz podatkov PubMed in PEDro. Rezultati: V analizo smo vključili 22 raziskav. Ugotovili smo zanesljive dokaze o učinkovitosti kinezioterapije pri zdravljenju TPL. Visokokakovostnih študij, ki bi potrdile učinkovitost uporabe ortoz, globinskih udarnih valov, infiltracijo zdravilnih učinkovin (s trombociti bogato plazmo, avtologne krvi in insulinu podobnem rastnem faktorju), dry needlinga, perkutane elektrolize in gliceril trinitrata, ni. Uporabnost: V sklopu fizioterapevtske obravnave pacientov s TPL priporočamo izvajanje kinezioterapije, ki vključuje izometrično, izotonično in pliometrično vadbo ter športno specifično vadbo. Omejitve: Ugotovitve večina raziskav temeljijo na majhnem vzorcu preiskovancev in kratkem trajanju študije, zato zanesljivih smernic ni mogoče oblikovati.

Secondary abstract: Introduction and purpose: Patellar ligament tendinopathy (TPL) is characterized by anterior knee pain. The most common cause is overloading caused by jumps. Conservative treatment should be firstly addressed. However, the efficiency of many physiotherapeutic treatment modalities is unknown. Methods: Systematic literature review was performed using the PubMed and PEDro electronic databases. Results: 22 studies were included. There is strong evidence that kinesiotherapy is effective for TPL treatment. However, there is no high-quality studies to confirm the effectiveness of orthoses, extracorporeal shock wave therapy, infiltration of protein rich plasma, autologous blood and insulin-like growth factor 1, dry needling, percutaneous electrolysis and glyceryl trinitrate. Usability: We recommend the kinesiotherapeutic treatment with isometric, isotonic, plyometric and sports specific exercises. Limitations: Due to the small number of people under investigation and short-term measurements, it is not possible to give reliable conclusions for the purpose of creating clinical guidelines.
