Abstract

Uvod: Presaditev jeter je za bolnike s kronično jetrno odpovedjo edini učinkovit način zdravljenja. Za učinkovitejše in hitrejše okrevanje po presaditvi je pomembna strokovna individualizirana in optimatizirana obravnava pacientov ter timsko delo zdravstvenega tima. V zaključnem delu smo želeli ugotoviti, katere komplikacije se največkrat pojavljajo pri pacientih po transplantaciji jeter v enoti intenzivne terapije. Metode: Izvedli smo sistematični pregled z metodo pregleda, analize in sinteze znanstvene literature ter metodo kompilacije. Strokovno literaturo smo iskali v podatkovnih bazah PubMed, MEDLINE, ScienceDirect in Google Scholar. Pregled literature smo predstavili s pomočjo PRISMA diagrama; vključene vire smo analizirali z uporabo vsebinske analize. Rezultati: V analizo identificiranih virov smo vključili osem raziskav. Med najpogostejše komplikacije po presaditvi jeter spadajo odpoved presadka, dihalna odpoved, okužba ter razvoj delirija v enoti intenzivne terapije. Pomembno vlogo za razvoj ter predvidevanje možnih zapletov po presaditvi imajo tudi dejavniki tveganja. Razprava in sklep: Pregled in analiza literature sta pokazala, da je kljub napredku perioperativne medicine pomembna postoperativna oskrba in multidisciplinarnost zdravstvenega tima. Menimo, da je na tem področju še vedno opravljenih premalo raziskav s področja zdravstvene nege in oskrbe teh pacientov, ki bi jih bilo priporočljivo izvesti ter s tem izboljšati kakovost obravnave pacientov po transplantaciji jeter.

Secondary title: Nursing care of a patient after liver transplantation
Secondary abstract: Introduction: Liver transplantation is the only effective treatment for a patient with chronic liver failure. Professional individualized and optimized treatment of patients and effective teamwork of the medical team are important factors for more efficient and faster recovery after transplantation. In the thesis we wanted to determine which complications most often occur in patients after liver transplantation in intensive care unit. Methods: We performed a systematic review using the method of review, analysis and synthesis of scientific literature and the compilation method. A literature search was conducted using PubMed, MEDLINE, ScienceDirect and Google Scholar. The review of the literature was presented using a PRISMA diagram; the included sources were analyzed using content analysis. Results: Eight studies were included in the analysis of identified sources. The most common complications after liver transplantation include graft failure, respiratory failure, infection and development of delirium in the intensive care unit. Risk factors also play an important role in developing and anticipating possible post-transplant complications. Disscussion and conclusion: The review and analysis of the literature has shown that despite the advances in perioperative medicine, postoperative care and the multi-disciplinarity of the medical team are important. In our opinion, there is still a lack of research in this area, which should be carried out to improve the quality of care of patients after liver transplantation.
