Abstract

Uvod: Ishemična vadba proti majhnemu uporu povzroča primerljiv ali celo večji prirast mišične zmogljivosti kot enaka vadba s prostim pretokom krvi, k čemur verjetno prispeva tudi izboljšana živčna aktivacija skeletnih mišic. Namen pregleda literature je bil ugotoviti vpliv ishemične vadbe proti majhnemu uporu na mišično aktivacijo. Metode: V podatkovnih zbirkah PubMed, PEDro, Cochrane Library in Science Direct smo iskali radomizirane in nerandomizirane kontrolirane raziskave s področja. Rezultati: V pregled je bilo vključenih enajst raziskav, v katerih so primerjali učinke ishemične vadbe proti majhnemu uporu in enake vadbe z normalnim pretokom krvi; v štirih raziskavah so v primerjavo vključili tudi vadbo proti velikemu uporu z normalnim pretokom krvi. Izsledki kažejo, da ob enakem mehanskem uporu in količini vadbe ishemična vadba pomembno bolj poveča aktivacijo skeletnih mišic kot vadba z normalnim pretokom krvi. Tako pa se tudi pospeši centralno in periferno utrujanje, kar je razvidno iz izrazitega upada tako ravni aktivacije centralnega živčevja kot vzdražnosti mišic takoj po ishemični vadbi. Opazen je tudi večji upad ravni aktivacije centralnega živčevja in vzdraženosti mišice med maksimalnim mišičnim naprezanjem takoj po ishemični vadbi, kar kaže na povečano centralno in periferno utrujanje v ishemičnih pogojih dela. Zaključki: Izsledki raziskav kažejo, da izvajanje vaj proti majhnemu uporu z oviranim pretokom krvi izboljša aktivacijo skeletnih mišic in pospeši utrujanje, vendar še ni jasno, ali to povzroči tudi dolgotrajne izboljšave.

Secondary abstract: Background: Low-load ischemic training is often used as an alternative to standard high-load resistance training. It is thought to elicit similar, or greater, gains in muscle size and strength at lower mechanical loads, which may be also attributed to enhanced neural activation of muscles. The purpose of this review was to determine the effects of low load ischemic resistance exercise on voluntary muscle activation. Methods: We reviewed PubMed, PEDro, Cochrane Library and Science Direct databases for relevant randomized and non-randomized controlled trials. Results: Eleven studies that compared the effects of low-load ischemic resistance exercise with the effects of equal low-load resistance exercise with free blood flow have been included and analysed. Four studies also used standard high-load exercise for comparison. The findings show that low-load ischemic resistance exercise can elicit significantly higher levels of skeletal muscle activation than standard exercise performed at the same intensity and volume. It also accelerates central and peripheral fatigue, which is evident from higher reduction of central activation level and muscle excitability immediately after ischemic exercise. Conclusions: Low-load ischemic resistance exercise can augment activation of skeletal muscles and accelerate fatigue, but it remains unknown if this elicits long-lasting improvements.
