Abstract
Uvod: Nenadni srčni zastoj je eden najpogostejših vzrokov smrti. Za preživetje je ključno izvajanje temeljnih postopkov oživljanja s stisi prsnega koša, ki morajo biti dovolj kakovostni. Kot možno nadomestilo ročnih stisov so se začeli uveljavljati mehanski stisi prsnega koša. V diplomskem delu smo primerjali ročno in mehansko izvedene stise prsnega koša ter vpliv na pacientovo preživetje.
Metode: Izvedli smo krovni (sistematični) pregled sistematičnih člankov na tematiko primerjave med uporabo mehanskih pripomočkov in ročne izvedbe stisov prsnega koša ter vplivov na klinične izide. Uporabili smo opisno metodo dela. Izbrane članke smo analizirali s pomočjo tabel in grafov.
Rezultati: Od 71 zadetkov smo podrobno analizirali 8 člankov, s skupnim vzorcem 143 999 oseb. Vsi sistematični pregledi so skupaj vključevali 102 študiji, oziroma 54 študij brez dvojnikov. Najpogosteje uporabljena mehanska pripomočka za izvajanje temeljnih postopkov oživljanja sta bila LUCAS (angl. Lund University Cardiac Arrest System) in AutoPulse. Skoraj vse študije so obravnavale mehanske stise prsnega koša izvajane pri zunajbolnišničnem srčnem zastoju, slaba polovica pa pri znotrajbolnišničnem srčnem zastoju.
Razprava in zaključek: Avtorji so med seboj neenotni glede najučinkovitejšega načina izvajanja stisov prsnega koša. Na splošno ni mogoče potrditi večje učinkovitosti izvajanja mehanskih stisov prsnega koša, saj obstajajo tudi razlike med samimi napravami in njihovim načinom delovanja. Ti so se za koristne izkazali v primeru zunajbolnišničnega srčnega zastoja, vendar obstajajo pomisleki glede preživetja in varne obravnave.

Introduction: Sudden cardiac arrest represents one of the most frequent causes of death. Patient survival depends on performing cardiopulmonary resuscitation with quality chest compressions. In the last decades, mechanical chest compressions are substituting manual chest compressions. The purpose of this study is to compare manual and mechanical chest compressions, resulting in patient survival.
Methods: We performed a systematic analysis of review articles about the comparison between the use of mechanical chest compression devices and manual chest compressions and their impact on patient survival. We used the descriptive method. The selected articles were analyzed using tables and graphs.
Results: Out of 71 hits, we analyzed eight articles with 143,999 subjects. All of the systematic reviews included 102 studies, to be precise, fifty-four of them without duplicates. Most frequently used devices for performing cardiopulmonary resuscitation were LUCAS (Lund University Cardiopulmonary Assist System) and AutoPulse. Almost every study is based on a research of mechanical chest compressions in Out-of-Hospital cardiac arrest; furthermore, less than half of the studies included a research of In-Hospital cardiac arrest.
Discussion and conclusion: Authors are still inconsistent about the most effective way to perform chest compressions. In general, it is not possible to confirm superiority of mechanical chest compressions to manual chest compressions due to the differences between device types and their mechanism. They are found to be useful in the case of Out-of-Hospital cardiac arrest with few concerns about patient survival and safety. |
