Abstract

Uvod: Oskrba ob koncu življenja je pomemben vidik zdravstvene nege, saj z njo zagotavljamo podporo tako umirajočemu kot njegovim najbližjim. Nudenje tovrstne oskrbe predstavlja vsaki medicinski sestri čustven napor, še posebno pa medicinski sestri na začetku kariere (< 36 mesecev delovne dobe v bolnišnici), ki se s tovrstnim delom srečuje prvič. Namen zaključnega dela je tako raziskati, kakšne izkušnje imajo medicinske sestre na začetku kariere pri zagotavljanju oskrbe ob koncu življenja pacienta v bolnišnicah. Metode: Izveden je bil pregled znanstvene literature v angleškem jeziku. Iskali smo po podatkovnih bazah CINAHL, PubMed, SAGE, ScienceDirect in Web of Sciences. Omejili smo se na članke, ki obravnavajo izbrano tematiko in brez časovnega okvira. Rezultati: Ugotovljeno je bilo, da medicinske sestre na začetku kariere smrt pacienta prizadene. Oskrba ob koncu življenja jih čustveno obremenjuje, občutijo nemoč, krivdo, jezo in žalost. Pomaga jim lahko pogovor s kolegi in samorefleksija. Največje težave pri zagotavljanju oskrbe ob koncu življenja jim predstavlja pomanjkanje znanja, izkušenj v podobnih situacijah, pomanjkljiva komunikacija in odsotnost pomoči s strani kolegov. Razprava in zaključek: Oskrba ob koncu življenja bi morala biti bolj obravnavano področje že v času študija, saj se z njo pri svojem delu srečujejo vse medicinske sestre. S samim preučevanjem izkušenj medicinskih sester na začetku kariere pri zagotavljanju oskrbe ob koncu življenja pa lahko izdelamo nove smernice izobraževanja, ki bi jih bolje pripravile na izzive oskrbe ob koncu življenja, posledično pa bi se dvignila tudi kvaliteta oskrbe, ki so je pacienti deležni.

Secondary abstract: Introduction: End-of-life care is important in nursing as it supports the dying and their loved ones. For every nurse providing this kind of care involves emotional labour, especially for nurses at the beginning of their career (< 36 months of work experience in a hospital) who do this kind of work for the first time. The aim of this diploma thesis is to investigate novice nurses’ experiences in providing end-of-life care for patients in hospitals. Methods: A review of scientific literature in the English language was conducted using the CINAHL, PubMed, SAGE, ScienceDirect, and Web of Science databases. The search was limited to articles dealing with the selected topic with no time boundaries. Results: It was found that nurses in the early stages of their careers are affected by the death of a patient. Providing end-of-life care imposes an emotional burden on them, they experience helplessness, guilt, anger, and sadness. Talking with colleagues and self-reflection can help them. The biggest problems in providing end-of-life care are lack of knowledge, lack of experience in similar situations, poor communication, and lack of support from colleagues. Discussion and conclusion: The topic of end-of-life care should be discussed already during studies, as all nurses deal with it at work. By studying the experiences of novice nurses in providing end-of-life care for patients, new education guidelines can be created that would better prepare nurses for the challenges of end-of-life care and, as a result, the quality of patient care would also increase.
