Abstract

Ruptura rotatorne manšete (v nadaljevanju: RM) je najpogostejši vzrok bolečine in omejitve funkcije ramenskega obroča. Pojavlja se tako pri mlajših, večinoma kot travmatska poškodba tetive RM, ali pri starejših, pri katerih prevladuje degenerativna preobremenitev tetive, ki posledično povzroči rupturo RM. Zdravnik se skupaj s pacientom, glede na velikost rupture in pacientove želje, odloči za operativno ali konzervativno zdravljenje. Najpogostejša intervencija po poškodbi RM, predvsem pri mlajših osebah in masivnih rupturah, je artroskopsko operativno zdravljenje. Ne glede na izbiro zdravljenja je pomembna čimprejšnja vključitev pacienta v program rehabilitacije. Glede na zdravniško pooperativno oceno in dejavnike tveganja se pacient vključi v program rehabilitacije, ki mu omogoča najbolj optimalen napredek v procesu okrevanja po operaciji RM. Zaradi pomembnosti rehabilitacije v procesu celjenja tetive po operativnem zdravljenju RM smo v diplomski nalogi predstavili ugotovitve učinkovitosti štirinajstdnevne zdraviliške rehabilitacije po operaciji RM. V raziskavo smo vključili 40 pacientov, ki so bili na rehabilitaciji v Diagnostično rehabilitacijskem centru Terme Zreče. Kot pokazatelj učinkovitosti štirinajstdnevne rehabilitacije smo uporabili Ocenjevalno lestvico za ramo Univerze Kalifornija, Los Angeles (UCLA Shoulder Rating Scale). Ugotovitve naše raziskave kažejo, da se bolečina po štirinajstdnevni zdraviliški rehabilitaciji na lestvici od 1 do 10 izboljša za 2,95 stopnje, kar je statistično značilno (p < 0,05). Prav tako smo ugotovili, da se gibljivost povprečne aktivne elevacije poveča s 103,9 stopinj na 131,3 stopinje, kar pomeni statistično značilno izboljšanje za 27,4 stopinje (p < 0,05). S potrditvijo obeh hipotez smo dokazali, da sta glavna cilja rehabilitacije izboljšanje gibljivosti in zmanjšanje bolečine ramenskega sklepa uspešno dosežena.

Secondary title: Evaluating the effectiveness of physiotherapeutic treatment after rotator cuff surgery
Secondary abstract: The rotator cuff (henceforth: RC) tear is the most common cause of pain and limited function of the shoulder girdle. It occurs to young people, mostly as a traumatic injury of the RC tendon and to old people as a degenerative overload of the tendon, consequently causing a tear of the RC. Depending on the size of the tear and the patient's wishes, the doctor decides for surgical or conservative treatment. The most common intervention after the RC injury, especially with younger people and masive tears is arthroscopic surgical treatment. Regardless of the choice of treatment, it is important to include the patient in the rehabilitation protocol as soon as possible. Considering the medical postoperative assessment and the risk factors, the patient is included in a rehabilitation program that allows him the most optimal progress in the recovery process after the surgery of the RC. Due to the importance of rehabilitation in the process of tendon healing after surgical treatment of the RC, we presented the findings of the fourteen-day spa rehabilitation effectiveness after the RC surgery. In the research we included 40 patients, who were in rehabilitation at the Diagnostično rehabilitacijski center Terme Zreče. As an indicator of effectiveness of the fourteen-day rehabilitation, we used the UCLA Shoulder Rating Scale. The findings of our research show that after fourteen days of spa rehabilitation on the scale from 1 to 10 improves for 2,95 levels, which is statistically significant (p < 0,05). We also found, that the mobility of the average active elevation increases from 103,9 to 131,3 degree of arc levels, which shows a statistically significant improvement for 27,4 degree of arc (p < 0,05). By confirming both hypotheses, we proved that the main goals of rehabilitation improving mobility and reducing pain were successfully achieved.
