Abstract

Uvod: V diplomskem delu sva predstavili ishemično možgansko kap in vlogo računalniške tomografije pri diagnosticiranju le-te. S predstavitvijo slikovnega protokola, ki vključuje angiografsko in perfuzijsko slikanje, sva podrobneje raziskali vlogo računalniške tomografije pri zgodnjem odkrivanju ishemične možganske kapi. Namen: Namen diplomskega dela je predstavitev kliničnega protokola CT slikanja pri ishemični možganski kapi ter opis in predstavitev kliničnega primera CT nevroperfuzije. Želeli sva ugotoviti, s katerimi parametri najučinkoviteje ocenjujemo prisotnost in stopnjo ishemične možganske kapi, ter katere so tehnične zahteve za kakovostno izvedbo preiskave. Metode dela: V diplomskem delu sva uporabili deskriptivno metodo oziroma opisno metodo dela in metodo s pregledom literature. Viri, ki sva jih pregledali so bili znanstveni članki, zborniki ter poglavja iz zbornikov in knjižni viri. Iskanje sva izvajali v podatkovnih bazah: Digitalna knjižnica Univerze v Ljubljani, Google Učenjak, Cobiss, ZLibrary, WebMD, PubMed in Radiopedia. Rezultati: Za lažji prikaz postopka sva na podlagi kliničnega primera pacienta predstavili protokol CT perfuzije pri ishemični možganski kapi. Rezultati perfuzijskih map so pokazali odsotnost penumbre, t.i. ishemičnega tkiva. Zaradi veličine infarktnega jedra pacient ni bil primeren za zdravljenje z reperfuzijo. Razprava in zaključek: Protokol, ki ga uporabljava se ni razlikoval s protokoli, ki jih uporabljajo v tuji literaturi. Ugotovili sva, da je med različnimi ustanovami vseeno prišlo do manjših odstopanj v samem zaporedju protokola, ki vpliva na prikaz kontrastnega sredstva v žilah. Parametri za oceno perfuzije so v literaturi opisani enako. Radiološke slikovno-preiskovalne metode so v nekaj letih zelo razvile in napredovale, saj nam današnja programska oprema nudi boljši prikaz in razmejitve med rešljivim tkivom in infarktom.

Secondary abstract: Introduction: The ischaemic stroke and the role of computed tomography in diagnosis of stroke are explained in this diploma thesis. By presenting an imaging protocol including angiographic and perfusion imaging, we further investigated the role of computed tomography in the early detection of the ischaemic stroke. Purpose: The purpose is to present the clinical protocol of CT imaging in ischaemic stroke and to describe a clinical case of CT neuro perfusion. We wanted to find out which parameters are the most effective in assessing the presence and severity of the ischaemic stroke, and what are the technical requirements for a good quality investigation. Methods: We used the descriptive method and the literature review method. The sources we looked at were scientific articles, proceedings and chapters from proceedings and book sources. We searched the University of Ljubljana Digital Library, Google Scholar, Cobiss, ZLibrary, WebMD, PubMed and Radiopedia. Results: To facilitate the demonstration of the procedure, we have presented a CT perfusion protocol for ischaemic stroke based on a patient case. The perfusion maps showed the absence of penumbra or ischaemic tissue. Due to the size of the infarct core, the patient was not suitable for reperfusion therapy. Discussion and conclusion: The protocol we used for neuroperfusion did not differentiate from those used in the foreign literature. We found out that there were slight variations between institutions in the actual sequence of the protocol, which affects the display of the contrast agent in the veins. The parameters used to assess perfusion are described in the same way as in the literature. Radiological imaging methods have evolved and advanced considerably over the years, as today's software gives us better visualization and delineation between salvageable tissue and infarction.
