Abstract

Uvod: Dojenje je naravni pojav, a vseeno v nekaterih primerih ne steče brez težav in bolečin. Eden izmed vzrokov, da se dojenje ne vzpostavi tako, kot bi se moralo, je vazospazem bradavic oziroma Raynaudov fenomen. Namen: Namen diplomskega dela je raziskati temo in ugotoviti, kakšne izzive Raynaudov fenomen bradavice prinaša ženski, ki si želi dojiti. Metode dela: Uporabili smo kvalitativni pristop raziskovanja. Literaturo smo našli na različnih podatkovnih bazah, kot so Cobiss, PubMed, Google Scholar, DiKul in Wiley Online Library. Iskanje literature je potekalo s pomočjo ključnih besed, navedenih v nadaljevanju. Izbor ustrezne literature je potekal glede na vsebinsko ustreznost člankov, dostopnost člankov v polnem besedilu in časovno ustreznost izdaje člankov. Izbrane članke smo ocenili po nivoju kakovosti s pomočjo hierarhije dokazov. Rezultati: V končno metaanalizo je bilo uvrščenih 12 člankov. Ugotovili smo, da simptomi vključujejo dvo- ali trifazne spremembe barve bradavice, prav tako pa se lahko pojavijo hude oziroma ostre bolečine kadar koli pred, med ali po dojenju. Vzrok za napad vazospazma bradavic zajema kompleksno interakcijo med genetskimi, živčnimi, žilnimi in intravaskularnimi dejavniki. Za prepoznavanje in pravilno diagnostiko Raynaudovega fenomena bradavice sta ključna dobra anamneza in fizični pregled, saj se Raynaudov fenomen bradavic pogosto napačno diagnosticira, kar vodi do neustreznega zdravljenja. Blage napade vazospazma je mogoče pogosto obvladovati z izogibanjem sprožilcem, kadar pri ženski pride do hudih napadov, pa se poslužujemo medikamentoznega načina zdravljenja. Bolečina pri dojenju je pogost razlog, zakaj ženske prenehajo dojiti, zato je treba težavo zgodaj prepoznati in pravilno ukrepati. Razprava in zaključek: Ugotovili smo, da je potrebno težavo pri dojenju pravilno identificirati in najti primeren način, kako k reševanju pristopiti. Kot zdravstveni strokovnjaki moramo biti dovolj izobraženi, da lahko ženski predajamo ustrezno znanje, jo naučimo samopomoči in ji nudimo ustrezno podporo.

Secondary abstract: Introduction: Breastfeeding is a natural phenomenon, but in some cases it is not without problems and pain. One of the reasons that breastfeeding is not established as it should be is nipple vasospasm or Raynaud's phenomenon. Purpose: Breastfeeding is a natural phenomenon, but in some cases it is not without problems and pain. One of the reasons that breastfeeding is not established as it should be is nipple vasospasm or Raynaud's phenomenon. Purpose: The purpose of the thesis is to investigate the topic and find out what challenges the Raynaud's phenomenon of the nipple brings to a woman who wants to breastfeed. Methods: We used a qualitative research approach. Literature was found on various databases such as Cobiss, PubMed, Google Scholar, DiKul and Wiley Online Library. The literature search was carried out using the keywords listed below. The selection of relevant literature was based on the relevance of the content of the articles, the availability of the full text of the articles and the timing of the publication of the articles. We evaluated the selected articles according to their level of quality using the evidence hierarchy. Results: 12 articles were included in the final meta-analysis. We have found that the symptoms include two or three phase changes in the color of the nipple, and severe or sharp pain can occur at any time before, during or after breastfeeding. The cause of an attack of nipple vasospasm involves a complex interaction between genetic, nervous, vascular and intravascular factors. A good medical history and physical examination are essential for the recognition and correct diagnosis of Raynaud's wart phenomenon, as Raynaud's wart phenomenon is often misdiagnosed, leading to inadequate treatment. Mild attacks of vasospasm can often be managed by avoiding the triggers, but when a woman has severe attacks, we use medication. Breastfeeding pain is a common reason why women stop breastfeeding, so it is important to recognize the problem early and act accordingly. Discussion and conclusion: We found that it is necessary to correctly identify breastfeeding problems and find a suitable way to approach the solution. As health professionals, we need to be sufficiently educated to be able to impart appropriate knowledge to the woman, teach her self-help and provide her with appropriate support.
