Abstract

Uvod: Transkateterska implantacija aortne zaklopke je perkutana tehnika, ki je postala priznana metoda izbire zdravljenja za simptomatsko hudo kalcifikacijsko aortno stenozo. Namen zaključnega dela je raziskati in prikazati vlogo medicinske sestre v intradisciplinarnem timu od začetka do konca obravnave bolnika pri transkateterski implantaciji aortne zaklopke. Metode: Izveden je bil pregled literature z deskriptivno metodo dela in metodo analize ter sinteze strokovne in znanstvene literature, z upoštevanjem smernic PRISMA. Literaturo smo iskali na osnovi zastavljenega raziskovalnega vprašanja v mednarodnih bazah podatkov PubMed, CINAHL, Sage in ScienceDirect. Ob tem smo upoštevali zastavljene vključitvene in izključitvene kriterije ter limite iskanja. Rezultati: V končno analizo je bilo vključenih pet člankov, ki so ustrezali iskalnemu nizu in odgovorili na naše raziskovalno vprašanje. Ugotovljeno je bilo, da je vloga medicinske sestre velika in odgovorna ter obsega številne dejavnosti in aktivnosti, zato je potreba po raziskovanju tega področja nujna. Razprava in sklep: Odgovornost medicinske sestre pri obravnavi bolnika v programu TAVI vedno bolj narašča. V predoperativnem obdobju sodeluje s kardiologom glede načrta in izvajanja diagnostično terapevtskega načrta, se seznani s pacientom in tudi njegovimi svojci. Pri operativnem izvajanju sodeluje izkušena operacijska medicinska sestra, ki ima posebna znanja in spretnosti glede izvajanja posega. Po posegu medicinska sestra na oddelku izvaja nadzor bolnika, predpisane terapevtske in diagnostične postopke ter skrbi, da je ob odpustu dovolj poučen o svojem zdravstvenem stanju.

Secondary abstract: Introduction: Transcatheter aortic valve implantation is a percutaneous technique that has become a recognized method of treatment for symptomatic severe calcific aortic stenosis. The purpose of the final work is to investigate and demonstrate the role of the nurse in the interdisciplinary team from the beginning to the end of the treatment of the patient during transcatheter implantation of the aortic valve. Methods: A review of the literature was carried out using a descriptive method of work and a method of analysis and synthesis of professional and scientific literature, taking into account the PRISMA guidelines. Literature was searched based on the research question in the international databases PubMed, CINAHL, Sage and ScienceDirect. At the same time, we took into account the set inclusion and exclusion criteria and search limits. Results: Five articles that matched the search string and answered our research question were included in the final analysis. It was found that the role of a nurse is large and responsible and includes many activities and activities, therefore the need to research this area is urgent. Discussion and conclusion: The responsibility of the nurse in treating the patient in the TAVI program is increasing more and more. In the pre-operative period, she cooperates with the cardiologist regarding the diagnostic and therapeutic plan. The nurse gets to know the patient and also his relatives. An experienced operating nurse, who has special knowledge and skills regarding the complexity of the intervention, participates in the operative implementation of the procedure itself. After the procedure, the nurse supervises the patient, performs the prescribed therapeutic and diagnostic procedures, and ensures that he is adequately informed about his health condition upon discharge.
