Abstract

Uvod: V razvitih državah sta se v zadnjem desetletju razširjenost in obseg izgube zob zelo zmanjšala, razširjenost brezzobosti pa je v zadnjih desetih letih padla za 4–10 %. Danes znaša življenjska doba ljudi že čez 80 let, zobozdravstvene ordinacije in klinike pa so vse bolj polne starejših pacientov. Življenje se podaljšuje, življenjska doba zob pa ne, zato se daljša obdobje nošenja totalne proteze. Kadar proteza ni pravilno izdelana, lahki primeri postanejo težki, težki pa še kaotičnejši. Namen: Namen diplomskega dela je pregled obstoječe literature o izdelavi totalne proteze po najnovejših računalniško podprtih metodah dela, od računalniško podprtega oblikovanja do računalniško podprte proizvodnje v zobnem laboratoriju. S tem želimo ugotoviti, ali sta oblikovanje in proizvodnja snemne protetike s subtraktivno metodo dovolj razvita za izdelavo totalne proteze. Protezo smo izdelali tudi sami in opis izdelave podkrepili s slikovnim gradivom. Metode dela: Teoretični del diplomskega dela je napisan po deskriptivni metodi, pri čemer smo informacije črpali iz strokovnih člankov in knjig iz knjižnice NUK, knjižnice na Medicinski fakulteti Univerze v Ljubljani, knjižnice na Zdravstveni fakulteti Univerze v Ljubljani in brskalnika Google Učenjak. V praktičnem delu pa je opisan laboratorijski postopek računalniško podprte izdelave totalne proteze po navodilih licenciranih programov, s katerimi smo pod nadzorom mentorja izdelali protezo. Rezultati: Izdelali smo totalno protezo z digitalnim subtraktivnim pristopom. V programu CAD smo na fantomskih modelih digitalno oblikovali celotno totalno protezo – od analize modela in postavitve zob do modeliranja dlesni. Datoteke smo nato izvozili v rezkalno napravo CAM, ki je iz diska PMMA izrezkala posamične zobe in bazo proteze. Zobe smo zalepili v luknje v bazi ter protezo še obdelali in spolirali do visokega sijaja. Razprava in zaključek: Potreba po oskrbi brezzobih pacientov bo vedno obstajala, morda se bo čez daljše časovno obdobje le zmanjšala. Totalna proteza, izdelana s subtraktivnim pristopom, ima veliko prednosti, zaradi visokih stroškov pa še vedno ni preferirana metoda izdelave. Sodobna tehnologija lahko zelo olajša delo v laboratoriju, vendar pa je na koncu še vedno vse odvisno od sposobnosti laboratorijskega zobnega protetika

Secondary title: Fabrication of a total prosthesis with a subtractive approach
Secondary abstract: Introduction: In developed countries, the prevalence and extent of tooth loss has decreased greatly over the past decade, and the prevalence of edentulism has fallen by 4-10 % over the past ten years. Nowadays, the life expectancy of people is already over 80 years and dental offices and clinics are becoming more and more filled with elderly patients. Although the lifespan is extended, the lifespan of the teeth is not, and therefore the period of wearing a total prosthesis is extended. When the prosthesis is not properly made, easy cases become difficult, and difficult cases become even more chaotic. Purpose: The purpose of this thesis is to review the already existing literature on the production of complete dentures using the latest computer-aided work methods, from computer-aided design to computer-aided production in the dental laboratory. With this, we want to find out whether the field of removable prosthetics, more specifically complete dentures, designed and produced with the subtractive method, is sufficiently developed for its production. We will make the denture ourself and support it with pictorial material. Methods: The theoretical part of the thesis was written using the descriptive method, where we drew information from professional articles and books that we found with the help of remote access to NUK libraries, the library at the Faculty of Medicine of the University of Ljubljana, the library at the Faculty of Health of the University of Ljubljana and Google Scholar. In the practical part, the laboratory process of computer-aided production of a complete denture is described, according to the instructions of the licensed programs with which we produced the denture, under the supervision of my mentor. Results: In our case, we made a complete denture using digital subtractive approaches. In the CAD program, we digitally designed the entire complete denture on phantom models, from model analysis, tooth placement to gum modeling. The files were then exported to a milling CAM device, which carved individual teeth and the base of the denture from the PMMA disc. The teeth were then glued into the holes in the base and finally the denture was processed and polished to a high shine. Discussion and conclusion: The need for the care of edentulous patients has never run out, at most it will decrease over a longer period of time. A complete denture made with a subtractive approach has many advantages, but due to high costs it is still not the preferred method of production. Modern technology can greatly facilitate the work in the laboratory, but in the end everything still depends on the skills of the laboratory dental prosthetist.
