Abstract
Uvod: Veščine vožnje vozička na aktivni pogon vplivajo na otrokovo zmožnost samostojnega premikanja, ki pri otroku lahko spodbuja tudi kognitivni in psihosocialni razvoj. Vloga delovnega terapevta je, da otroku skozi igro in na poenostavljen način predstavi voziček ter ga nauči učinkovitega ravnanja z njim. Programi učenja vožnje vozička so za otroško populacijo redko prilagojeni. Namen: V delu smo želeli oblikovati nabor aktivnosti za učenje vožnje vozička za otroke in njegovo uporabo prikazati na primeru deklice. Zanimale so nas spremembe v izvedbi ter zadovoljstvu deklice po uporabi nabora aktivnosti. Metode dela: Raziskava je bila izvedena v obliki singularne študije primera. V dvotedensko obravnavo je bila vključena petletna deklica, ki je mesec dni pred obravnavo prejela svoj prvi voziček na aktivni pogon. Ob sprejemu smo izvedli Oceno vožnje z vozičkom za otroke, s katero smo ugotovili, pri katerih veščinah ima deklica največ težav. V procesu delovne terapije smo uporabili deklici prilagojen nabor aktivnosti, s katerim smo želeli vplivati na izvedbo veščin. Za informacije ob sprejemu in prikaz napredka smo poleg ocene vožnje z vozičkom za otroke uporabili ocenjevanja: Kanadski test izvedbe okupacije (COPM), Lestvico doseganja ciljev (GAS), Analizo aktivnosti po spretnostih, Vprašalnik za oceno funkcijskih sposobnosti otrok (PEDI) ter opravili meritev stiska jakosti pesti z dinamometrom. Rezultati: Ponovna Ocena vožnje z vozičkom za otroke je pokazala 11-odstotno izboljšanje dosežka v primerjavi z začetno oceno. Deklica je pri izbranih postavkah, vključenih v nabor aktivnosti, dosegla višje število točk. Razlika v zadovoljstvu z izvedbo izbranih postavk je po Kanadskem testu izvedbe okupacije znašala 3,3. Deklica je povprečno izvedbo pri drugem ocenjevanju v COPM-u ocenila s 3,7 točke višjo oceno. Napredek za aktivnost prečkanje praga višine dveh centimetrov ter presedanje z desko smo prikazali z Lestvico doseganja ciljev, kjer je pri prvi aktivnosti napredovala od ocene –2 do ocene +2, pri drugi pa od ocene –2 do ocene +1. Razprava in zaključek: Rezultati govorijo v prid uspešnosti uporabljenega nabora aktivnosti pri učenju izbranih veščin. Deklica je bila po opravljenem programu vadbe bolj zadovoljna z izvedbo veščin. Nabor aktivnosti se je izkazal za učinkovito metodo vadbe.

Set of activities for learning how to drive an active wheelchair for children - a study case |

Introduction: Set of skills that allows a child to drive an active wheelchair, influences on his capability of independent mobility, which can also enhance cognitive and psychosocial development. The role of occupational therapist is to present and teach the effective use of an active wheelchair through interesting and playful approach. Programs for learning how to drive a wheelchair are rarely adapted for pediatric population. Purpose: We wanted to design a set of activities to enhance wheelchair driving skills and demonstrate it through a study case of a five year old girl. We were interested in changes with girl's perception of performance and satisfaction after we used our set of activities. Methods: Our research was conducted in a form of a singular study case. A 5 year-old girl, that recieved her first active wheelchair a month ago, was included in a two-week intervention. In the evalution phase we conducted an Assessment of wheelchair skills for children, which showed us the skills the girl had the most problems with. In the process of occupational therapy we used specially designed set of activites in order to improve girl's performance of skills. For gathering information about the girl and keeping track of the progress we used COPM, Goal assessment scale (GAS), Activity analysis, Pediatric Evaluation of Disability Inventory (PEDI). Inthe evaluation phase we measured girl's grip strength with a dynamometer. Results: After the intervention we repeated the Assesment of wheelchair skills for children, which has shown an 11 % progress. She achieved higher results in all of the chosen items included in set of activites. COPM showed an increase of 3,7 in performance and 3,3 in satisfatciton. We kept track of improvement and shown it with the Goal assessment scale. The girl has improved from level -2 to level +2 in the first activity and from level -2 to level +1 in the second activity that we measured. Discussion and conclusion: Results have shown positive impact of using our set of activities. Girl's performance and satisfaction with wheelchair skills after the intervention were higher than before. Our set of activities proved itself as an efficient method of learning a child how to drive a wheelchair. |
