Abstract

Uvod: Zadostna oksigenacija je pri kritično bolnih, okuženih s koronavirusno boleznijo 2019, za preživetje izrednega pomena. Pogosto sta za zagotavljanje optimalne oksigenacije potrebni endotrahealna intubacija in invazivna mehanska ventilacija. Pomembno je, da se s posameznimi aktivnostmi in intervencijami za izboljšanje oksigenacije pri neintubiranih kritično bolnih zmanjša potreba po endotrahealni intubaciji in invazivni mehanski ventilaciji. V zaključnem delu želimo predstaviti najpogosteje uporabljene intervencije za izboljšanje oksigenacije pri neintubiranih kritično bolnih s koronavirusno boleznijo 2019. Metode: V zaključnem delu smo uporabili opisno oziroma deskriptivno metodo dela. Prav tako smo uporabili metode analize, kompilacije, komparacije in sinteze. Literaturo smo poiskali v podatkovnih bazah PubMed, CINAHL Ultimate, Cochrane Library in Google Scholar. Iskanje literature smo prikazali po priporočilih PRISMA. Rezultati: V končno analizo smo vključili 18 raziskav. Najpogosteje uporabljene intervencije za izboljšanje oksigenacije pri neintubiranih kritično bolnih s koronavirusno boleznijo 2019 so: (1) obračanje neintubiranega pacienta na trebuh, (2) uporaba neinvazivne mehanske ventilacije, (3) aplikacija inhalacijskega dušikovega oksida, (4) nosna kanila z visokim pretokom. Razprava in zaključek: Potreba po endotrahealni intubaciji in invazivni mehanski ventilaciji se lahko ob pravočasni uvedbi intervencij za izboljšanje oksigenacije bistveno zmanjša.

Secondary title: Interventions to improve oxygenation in non-intubated critically ill patients with COVID-19
Secondary abstract: Introduction: Sufficient oxygenation is crucial for the survival of critically ill patients infected with COVID-19. Endotracheal intubation and invasive mechanical ventilation are often necessary to ensure optimal oxygenation. It is important that individual activities and interventions aimed at improving oxygenation in non-intubated critically ill patients reduce the need for endotracheal intubation and invasive mechanical ventilation. In the final thesis we want to present the most common interventions aimed at improving oxygenation in non-intubated critically ill patients with COVID-19. Methods: In the final thesis we used the descriptive work method. The following methods were also used: analyses, compilations, comparisons and syntheses. Our sources were selected from the databases PubMed, CINAHL Ultimate, Cochrane Library and Google Scholar. The selection of literary sources was carried out in line with the PRISMA recommendations. Results: Eighteen studies were included in the final analysis. The most common interventions aimed at improving oxygenation in non-intubated critically ill patients with COVID-19 are: (1) positioning the non-intubated patient on the stomach, (2) non-invasive mechanical ventilation, (3) administration of inhaled nitric oxide, (4) application of a high-flow nasal cannula. Discussion and conclusion: With timely interventions aimed at improving oxygenation the need for endotracheal intubation and invasive mechanical ventilation can be significantly reduced.
