Abstract

Mišica piriformis je ploščata mišica hruškaste oblike, ki leži v globokem glutealnem predelu neposredno pod veliko zadnjično mišico, sindrom mišice piriformis pa je klinično stanje, za katerega so značilni simptomi, ki so posledica stiskanja oziroma draženja ishiadičnega živca zaradi mišice piriformis. Simptomi, ki se pojavijo pri pacientih s sindromom mišice piriformis, so predvsem bolečina v zadnjici, ki seva v kolk, zadnjo stran stegna in zgornji del spodnjega dela noge. Sindrom mišice piriformis je treba vključiti v diferencialno diagnozo pri vseh pacientih z bolečino v glutealnem predelu, predvsem kadar diagnostične preiskave ne potrdijo patologij ledvene hrbtenice in kolčnega sklepa. Diagnosticiranje sindroma mišice piriformis je zaradi njegove nejasnosti težko in temelji predvsem na anamnezi in kliničnem pregledu, na koncu pa se, če je to le mogoče, izvedejo diagnostične preiskave, kot so CT, MRI, ultrazvok in EMG. K zdravljenju sindroma mišice piriformis prištevamo predvsem konzervativne možnosti fizikalne terapije, uporabo protivnetnih sredstev, injekcije lokalnih anestetikov in kortikosteroidov ter novejšo metodo zdravljenja z injekcijo botulinskega nevrotoksina. Če nobena od naštetih metod zdravljenja ni uspešna, se svetuje kirurško zdravljenje. Najpogosteje uporabljeni fizioterapevtski postopki pri zdravljenju sindroma mišice piriformis so ultrazvočna terapija, manualna terapija mehkih tkiv, raztezanje, hlajenje, površinsko segrevanje ter funkcionalna vadba. V diplomskem delu smo s pregledom literature uspešno predstavili fizioterapevtsko obravnavo pacienta s sindromom mišice piriformis in s pomočjo raziskav, ki kažejo dobre rezultate, dokazali, da so predstavljene metode in tehnike učinkovite pri zdravljenju sindroma mišice piriformis.

Secondary abstract: The piriformis muscle is a flat, pear-shaped muscle located in the deep gluteal region directly under the gluteus maximus. Piriformis syndrome is a clinical condition characterised by symptoms resulting from compression or irritation of the sciatic nerve by piriformis muscle. The symptoms that occur in patients with piriformis syndrome are mainly buttocks pain radiating to the hip, the back of the thigh and the upper part of the lower leg. Piriformis syndrome should be included in the differential diagnosis in all patients with gluteal pain, especially when diagnostic investigations fail to confirm pathologies of the lumbar spine and hip joint. Diagnosing piriformis syndrome is difficult due to its vagueness and is based mainly on history and clinical examination, but ultimately diagnostic tests such as CT, MRI, ultrasound and EMG are performed whenever possible. The treatment of piriformis syndrome mainly includes conservative physical therapy options, anti-inflammatory agents, injections of local anaesthetics and corticosteroids, and the newer treatment method of botulinum neurotoxin injection. If none of these treatments are successful, surgery is advised. The most commonly used physiotherapy treatments for piriformis syndrome are ultrasound, manual soft tissue therapy, stretching, cryotherapy, surface heating and functional exercise. In this graduation thesis, we have successfully presented the physiotherapeutic treatment of patients with piriformis syndrome through a review of the literature. With the help of studies showing good results, we have demonstrated that the methods and techniques presented are effective in the treatment of piriformis syndrome.
