Abstract

Okvara obraznega živca je bolezensko stanje, ki nastane zaradi utesnitve, najpogosteje v dolgem kostnem kanalu. Na motorični strani se pojavi značilna akutna, enostranska in delna ali popolna paraliza obraznih mišic, kar pomeni izpad hotenih gibov ene strani obraza in prizadene številne funkcije, kot so hranjenje, obrazna mimika, zapiranje vek, govor, ter bistveno zmanjša kakovost življenja, vpliva pa prav tako na socialno in psihično stanje obolelega. Bellova pareza je idiopatska okvara obraznega živca perifernega tipa. Izraz idiopatski pomeni obolenje brez specifičnega vzroka. Do okvare pride zaradi vnetja obraznega živca na genikulatnem gangliju, zadebelitve živca in kompresije. Pojavnost Bellove pareze znaša približno 20 prizadetih na 100.000 oseb na leto. Kar 85 odstotkov obolelih z Bellovo parezo popolnoma okreva v roku treh mesecev. Med težje posledice okvare uvrščamo povišan tonus, masovne gibe in sinkinezijo, zaradi katerih je normalna obrazna mimika trajno motena. Pravilno in pravočasno zdravljenje tovrstne okvare lahko veliko pripomore k uspešnemu izidu zdravljenja. Ob pojavu simptomov se pacientom priporočajo mirovanje, počitek ter izogibanje prepihu in vetru. V akutni fazi (do 72 ur) zdravljenje temelji na kortikosteroidih in protivirusnih zdravilih. Pomembna je zaščita očesa v primeru nezmožnosti zapiranja vek. Med relativno nove metode za zdravljenje sodi elektroakupunktura, ki po rezultatih vključenih študij velja za varno metodo, ki temelji na električni stimulaciji. Metoda deluje po mehanizmu blokiranja bolečinskih dražljajev z aktiviranjem različnih bioaktivnih snovi preko perifernih, spinalnih in supraspinalnih mehanizmov. Akupunkturne točke so enake kot pri tradicionalni akupunkturi. EA v kombinaciji s farmakološkim zdravljenjem bistveno izboljša rezultate zdravljenja. Po pregledu študij lahko potrdimo učinkovitost metod EA in LILLT na končni izid zdravljenja in odpravo simptomov. EA je učinkovita in varna metoda za zdravljenje Bellove pareze in izboljša končni izid zdravljenja.

Secondary title: Efficacy of electro acupuncture in facial nerve palsy
Secondary abstract: Facial nerve palsy is a condition caused by compression of the facial nerve in the long bony canal. On the motor side, there is a characteristic acute, unilateral and partial or complete paralysis of the facial muscles, which means that voluntary movements of one side of the face are lost, and affects many human functions such as feeding, facial expressions, eyelid closure, speech, and significantly reduces the quality of life, as well as the social and psychological state of the sufferer. Bell's palsy is an idiopathic facial nerve disorder of the peripheral type. The defect occurs as a result of inflammation of the facial nerve at the geniculate ganglion, thickening of the nerve and compression. The incidence of Bell's palsy is 20/100 000 persons per year. As many as 85 % of Bell's palsy recovers completely within three months. The more severe consequences of the defect include increased tone, mass movements and synkinesia, which permanently disrupt normal facial expressions. Correct and timely treatment of this type of defect can make a significant contribution to a successful outcome. At the onset of symptoms, patients are advised to rest, remain still and avoid draughts and wind. In the acute phase (up to 72 hours), treatment is based on corticosteroids and antiviral drugs. Eye protection is important in case of inability to close the eyelids. Relatively new methods of treatment include electroacupuncture, which, according to the results of the studies included, is considered a safe method based on electrical stimulation. The method works by blocking pain stimuli through the activation of various bioactive substances via peripheral, spinal and supraspinal mechanisms. The acupuncture points are the same as in traditional acupuncture. EA combined with pharmacological treatment significantly improves the treatment results. After reviewing the studies, we can confirm the effectiveness of EA and LILLT on the final outcome of treatment and symptom resolution. EA is an effective and save method for the treatment of Bell's palsy and improves the final outcome of treatment.
