Abstract

Uvod: V sedanjem času je psihiatrična zdravstvena nega predvsem proces strokovne komunikacije in značilen medoseben odnos med medicinsko sestro in pacientom. Usmerjena je v pomoč pacientu, da doseže največjo mero samostojnosti, kar se dosega z individualno načrtovano zdravstveno nego, ki med drugim pacienta spodbuja k prevzemanju odgovornosti za lastno zdravje. Namen: Namen magistrske naloge je predstaviti značilnosti zdravstvene nege pacientov, vključenih v Skupnostno psihiatrično obravnavo Zdravstvenega doma Sevnica, in preveriti njihovo zadovoljstvo z obravnavo. Na podlagi študije primera bomo predstavili ta proces in tudi koordinacijo obravnave s strani diplomirane medicinske sestre. Metode dela: V magistrski nalogi smo uporabili kvantitativno in kvalitativno raziskovalno metodo. Kvalitativni del je študija primera pacientke s hudo duševno motnjo. Za zbiranje podatkov smo uporabili tehnike intervjuja, opazovanja ter pregled medicinske dokumentacije. Kvantitativni del je analiza zadovoljstva pacientov z obravnavo pri diplomirani medicinski sestri. Uporabili smo standardizirani vprašalnik za merjenje izkušenj pacientov z obravnavo (Patient Reported Experience Measures), z izbranimi vprašanji. Raziskovalni vzorec je dvajset pacientov, vključenih v Skupnostno psihiatrično obravnavo. Gre za naključen vzorec: izbrali smo vsakega desetega pacienta s seznama obravnavanih. Rezultati: Na podlagi študije primera smo ugotovili negovalne probleme. Na tej podlagi smo naredili načrt zdravstvene nege in načrtovali obravnavo. Iz vprašalnika za merjenje izkušenj pacientov z obravnavo smo ugotovili, da so pacienti večinoma zadovoljni z obravnavo. Najpogostejši diagnozi pri raziskovani skupini sta paranoidna shizofrenija in neopredeljena demenca. Najpogostejši postopek zdravstvene nege v Skupnostni psihiatrični obravnavi Posavje je bila zdravstvena vzgoja pacientov in svojcev. Razprava in zaključek: Značilnosti zdravstvene nege pri psihiatričnih pacientih so v Sloveniji pomanjkljivo raziskane in to raziskovanje je treba okrepiti, med drugim tudi glede komunikacijskih veščin. Obravnava pacienta po enajstih funkcionalnih vzorcih zdravega obnašanja po Marjory Gordon omogoča celovito zdravstveno negovalno obravnavo in jo zato priporočamo v uporabo v drugih timih skupnostne psihiatrične obravnave. Zadovoljstvo pacientov s storitvami skupnostne psihiatrične obravnave Zdravstvenega doma Sevnica je visoka, zato je metode dela in komunikacijske veščine, ki smo jih razvili v letih razvoja programa, treba prenesti tudi v druge skupnostne psihiatrične obravnave.

Secondary abstract: Introduction: Psychiatric health care today is primarily a process of professional communication and a distinctive interpersonal relationship between nurse and patient. They are oriented towards helping the patient to achieve a maximum degree of autonomy, which is achieved through individually planned health care that, among other things, encourages the patient to take responsibility for his or her own health. Purpose: The aim of the master thesis is to present the characteristics of the health care provided to the patients enrolled in the Community Psychiatric Treatment of the Sevnica Health Care Centre and to examine their satisfaction with the treatment. We will use a case study to present this process, as well as the coordination of care by a registered nurse. Methods: Quantitative and qualitative research methods were used in this thesis. The qualitative part is a case study of a patient with a severe mental disorder. Interview techniques, observation and review of medical records were used to collect data. The quantitative part is an analysis of patients' satisfaction with their treatment by a registered nurse. We used a standardized questionnaire to measure patient experience Patient Reported Experience Measures with selected questions. The research sample is twenty patients enrolled in the Community Psychiatric Treatment. It is a random sample: we selected every tenth patient from the list of patients treated. Results: We identified care problems based on the case study. Based on this, we made a nursing care plan and planned treatment. From the Patient Reported Experience Measures questionnaire, we found that patients were mostly satisfied with the treatment. The most common diagnoses in the study group were paranoid schizophrenia and unspecified dementia. The most common health care process in the Community Psychiatric Treatment was health education of patients and relatives. Discussion and conclusion: The characteristics of health care for psychiatric patients are under-researched in Slovenia and this research needs to be strengthened, including with regard to communication skills. The treatment of the patient according to Marjory Gordon's Eleven Functional Patterns of Healthy Behaviour provides a holistic nursing approach and is therefore recommended for use in other Community Psychiatric Treatment teams. Patient satisfaction with the services provided by the Sevnica Health Care Centre is high, and the working methods and communication skills developed over the years of the programme's development should be transferred to other Community Psychiatric Treatment.
