Abstract

Poškodbe rotatorne manšete (RM) pri športnikih nastanejo zaradi velikih obremenitev ramenskega sklepa pri kontaktnih športih in športnikih z značilnimi gibi rok nad višino glave, kjer abdukcija skupaj z zunanjo rotacijo negativno vpliva predvsem na mišico supraspinatusa. Pojavijo se kot posledica akutnih poškodb, kot so neposredni udarci, padci na iztegnjeno roko ali kronična prekomerna uporaba in ponavljajoče se mikrotravme. Za postavitev diagnoze ima ključno vlogo klinični pregled z anamnezo, inspekcijo, palpacijo, oceno aktivnega in pasivnega obsega gibanja, oceno mišične moči, diagnostičnim slikanjem ter kliničnimi provokacijskimi testi. Slikanje z magnetno resonanco (MRI) in ultrazvok (UZ) imata podobno občutljivost in specifičnost za oceno nativne RM. S fizioterapevtsko obravnavo želimo zmanjšati bolečino in vnetje, izboljšati gibljivosti glenohumeralnega (GH) sklepa ter izboljšati stabilnost, moč in živčno-mišični nadzor. Zdravljenje poteka konzervativno in vključuje celovit rehabilitacijski program, protivnetna zdravila in injiciranje kortikosteroidov. Fizioterapevtske metode in tehnike za zdravljenje poškodb RM pri športnikih so kinezioterapija, mobilizacija sklepa, terapija s kineziološkimi trakovi, termoterapija, terapija s suhim iglanjem, terapija z uporabo laserja in ultrazvoka. Pri športnikih s tendinopatijo s sočasno delno raztrganino ali brez nje, ki se na ostale metode konzervativnega zdravljenja ne odzivajo, priporočamo tudi terapijo z globinskimi udarnimi valovi. Operativno zdravljenje je rezervirano le za športnike, pri katerih konzervativno zdravljenje ni bilo uspešno. V diplomskem delu smo s pregledom strokovne literature predstavili učinkovitost fizioterapevtske obravnave pri zdravljenju poškodb RM športnikov in s pomočjo raziskav, ki navajajo dobre rezultate, potrdili, da so predstavljene fizioterapevtske metode in tehnike učinkovite pri zdravljenju poškodb RM športnikov.

Secondary title: Effectiveness of physiotherapeutic treatment in athletes with rotator cuff injuries
Secondary abstract: Rotator cuff (RC) injuries in athletes are caused by high loads on the shoulder joint in contact sports and in athletes with typical arm movements above head height, where abduction combined with external rotation has a negative impact mainly on the supraspinatus muscle. They occur as a consequence of acute injuries such as direct blows, falls on the outstretched arm, or chronic overuse and repetitive microtrauma. Clinical examination with history, inspection, palpation, assessment of active and passive range of motion, assessment of muscle strength, diagnostic imaging and clinical provocation tests play a key role in the diagnosis. Magnetic resonance imaging (MRI) and ultrasound (US) have similar sensitivity and specificity for the assessment of native RC. Physiotherapy treatment aims to reduce pain and inflammation, improve glenohumeral (GH) joint mobility and improve stability, strength and neuromuscular control. Treatment is conservative and includes a comprehensive rehabilitation programme, anti-inflammatory drugs and corticosteroid injections. Physiotherapy methods and techniques for the treatment of RC injuries in athletes include kinesiotherapy, joint mobilisation, kinesiology tape therapy, thermotherapy, dry needling, laser therapy and ultrasound. For athletes with tendinopathy with or without concomitant partial tear who do not respond to other conservative treatment methods, we also recommend deep shock wave therapy. Operative treatment is reserved only for athletes in whom conservative treatment has failed. In this thesis, we have reviewed the literature to present the effectiveness of physiotherapy in the treatment of RC injuries in athletes and, with the help of studies reporting good results, we have confirmed that the physiotherapy methods and techniques presented are effective in the treatment of RC injuries in athletes.
