Abstract
V članku skušamo odgovoriti na vprašanje, kako nekatere zunanje in notranje omejitve zmanjšujejo nabor odločitev, ki jih imajo na voljo poslovni direktorji javnih bolnišnic v Sloveniji. Analizirali smo vpliv kolektivnih pogodb (znotraj plačnega sistema in način kompenzacije za tveganja, ki izhajajo iz opravljanja zdravstvenega poklica) ter notranjo organizacijo javnih bolnišnic. Pokazali smo, da sta oba elementa omejevalna dejavnika pri racionalizaciji javnih sredstev za zdravstvo ter da zmanjšujeta koristi bolnikov, preko tega pa povečujeta zdravstvena tveganja oziroma zmanjšujeta možnost dostopa bolnikov do zdravstvenih storitev. Ugotavljamo, da bo za racionalizacijo javnega zdravstva potrebno povezati plačni sistem s produktivnostjo, organizacijsko shemo javnih bolnišnic pa spremeniti v organizacijsko shemo, ki je običajna za gospodarske družbe, v katerih se zaradi natančno določene razdelitve obveznosti in odgovornosti minimizirajo možnosti notranjih konfliktov.

Secondary abstract: |
The paper addresses the issue of internal and external constraints, faced by the managers of Slovenian hospitals. We analyze the influence of collective bargaining in Slovenian health care industry and the internal organization of public hospitals. We show that both elements, collective bargaining and the internal organization, constrain a more rational use of public resources devoted to healthcare and that they reduce the welfare of patients, increase risks and reduce access to health services. In order to improve the efficiency of the public health sector, it is necessary to link salaries to productivity and transform the internal organization of public hospitals into that of private enterprises. Such transformation would reduce the likelihood of internal conflicts due to precisely demarcated responsibilities of management and staff. |
