Abstract

Izhodišča: Rak vstopi v življenje pacienta in njegove družine nepričakovano ter poseže na vse ravni življenja. V takšnih primerih lahko optimalne možnosti zdravljenja in zdravstveno nego zagotavljamo samo s celostno obravnavo že od diagnostične faze, preko vseh faz zdravljenja in okrevanja. Glavni cilj zdravljenja Hodgkinovega limfoma je doseči ozdravitev, vendar je pot do ozdravitve težka, saj mora pacient skozi agresivno kemoterapijo in radioterapijo, ki puščata veliko neželenih učinkov. Namen: Namen diplomskega dela je predstaviti Hodgkinov limfom, ter s pomočjo študije primera prikazati pomen zdravstvene nege in vlogo medicinske sestre pri obravnavi pacienta s Hodgkinovim limfomom. Raziskovalna metodologija: V diplomskem delu je bila uporabljena opisna oziroma deskriptivna metoda dela. Raziskava je temeljila na kvalitativni metodologiji, izvedli smo študijo primera pacienta s Hodgkinovim limfomom. Obravnava pacientke je potekala na njenem domu. Informacije o pacientki smo pridobili na podlagi vprašalnika, ki je delno strukturirane narave, prav tako smo podatke pridobili iz njene medicinske in negovalne dokumentacije. Rezultati: Na podlagi študije primera smo ugotovili, da ima pacient s Hodgkinovim limfomom največ težav zaradi samega zdravljenja s kemoterapijo in radioterapijo. Najpogostejši negovalni problemi, ki jih imajo pacienti s Hodgkinovim limfomom so: težko dihanje in kašelj, težko in boleče požiranje, socialna izolacija zaradi zmanjšane imunosti, slabše splošno počutje, depresija, pomanjkanje samozavesti zaradi izgube las, slabše spanje zaradi bolečin, izpostavljenost infekcijam zaradi zmanjšane imunosti ter strah pred prognozo in posledicami zdravljenja. Sklep: Pacienti s Hodgkinovim limfomom morajo upoštevati navodila, ki jih daje njihov onkolog, saj s tem lahko preprečijo marsikatero težjo obliko neželenih učinkov citostatskega zdravljenja in obsevanja. Medicinska sestra v tem primeru deluje zdravstveno-vzgojno ter pacienta poučuje in mu svetuje glede na njene kompetence.

Secondary abstract: Introduction: Cancer enters patient’s life unexpectedly and interferes with all levels of his and his family’s life. Optimal treatment and care can be provided only with comprehensive treatment from the diagnostic phase, through all stages of treatment and recovery. The main goal of treating Hodgkin's lymphoma is to achieve patient's complete recovery. The path to recovery however is extremely difficult, since the patient has to go through aggressive chemotherapy and radiotherapy with many side effects. Purpose: The purpose of this study is to introduce Hodgkin's lymphoma and with a case study show the importance of health care and the role of nurses in the treatment of patients with Hodgkin's lymphoma. Research Methodology: This thesis uses a descriptive methodology. The survey was based on a qualitative methodology. We conducted a case study of a patient with Hodgkin's lymphoma. Treatment was held at the patient’s home. Information about the patient was obtained on the basis of a questionnaire, which is partially structured. These data were obtained from patient’s medical and nursing documentation. Results: Based on the case study we found that a patient with Hodgkin's lymphoma suffers the most from side effects of chemotherapy and radiotherapy. The most common nursing problems by patients with Hodgkin's lymphoma are: shortness of breath, cough, difficult and painful swallowing, social isolation due to reduced immunity, general distress, depression and lack of self-confidence due to hair loss, poor sleep due to pain, exposure to infections due to reduced immunity and fear of prognosis and consequences of the treatment. Conclusion: Patients with Hodgkin's lymphoma must follow instructions given by their oncologist, as this can prevent many severe side effects of cytostatic therapy and radiation therapy. The nurse in this case offers the patient health care and guides the patient through treatment in accordance to her competence.
