Abstract

Teoretična izhodišča: Možganska kap je še vedno eden resnih problemov sodobne medicine, tako zaradi velikega števila oseb z boleznimi možganskih žil kot tudi zaradi socialno-ekonomskih posledic možganske kapi, ki prizadenejo družino in celotno družbo. Tromboliza znotraj štirih ur in pol je uveljavljeno, učinkovito in vzročno zdravljenje ishemične možganske kapi. Vloga medicinske sestre pri trombolitičnem zdravljenju je pomembna in raznolika. Bolniku nudi strokovno pomoč vse od diagnostičnih postopkov dalje, nudi mu čustveno podporo in spodbudo med zdravljenjem. Je nepogrešljivi člen pri obravnavi bolnika po možganski kapi. Raziskovalna metodologija: Kot raziskovalni inštrument smo uporabili intervju, ki je vseboval 11 vprašanj odprtega tipa. V raziskavo je bil vključen en bolnik z možgansko kapjo na nevrološkem oddelku univerzitetnega kliničnega centra. Intervju smo opravili na dan sprejema v bolnišnico in ponovno tretji dan po opravljeni trombolizi. Rezultati: Rezultati raziskave so pokazali negovalne probleme, kot so: dihanje, neučinkoviti vzorci dihanja, samonega, zmanjšana zmožnost za samostojno osebno higieno, nepopolno požiranje in nevarnost aspiracije, urin, nekontrolirano izločanje, obstipacija in črevesna inkontinenca, nepopolna verbalna komunikacija in nevarnost padcev pred trombolitičnem zdravljenjem. Nevrološka prizadetost bolnika po trombolitičnem zdravljenju je bila bistveno manjša, prav tako so bili odpravljeni negovalni problemi. Sklep: Pri trombolizi ishemične možganske kapi je pomembno, da bolnik čim prej pride v center, ki je usposobljen za tovrstno zdravljenje. Medicinska sestra z vpogledom v posebnosti zdravstvene nege je pomemben član tima za obravnavo bolnikov z možgansko kapjo. Prepoznati mora značilne probleme v zdravstveni negi, jih oceniti, slediti in obvladovati ter s tem zmanjšati njihov vpliv na bolnika.

Secondary title: The role of nurses in thrombolytic treatment of stroke
Secondary abstract: Background: Stroke is still a major health care issue due to high prevalence of cerebrovascular diseases and disability related large socio-economic burden for the patient, his family and society. Ischemic stroke caused by a blood clot can be very effectively treated by clot lysis i.e. thrombolysis. Thrombolysis within four and a half hours is an established, effective and causal treatment for ischemic stroke. Nurse plays an important and diverse role in stroke thrombolysis. She offers professional assistance from the diagnostic procedures on, gives emotional support and encouragement during treatment. Nurse is an indispensable member of the stroke team. Methods: We have used a questionnaire with eleven open type questions in one patient with stroke treated at the neurology department at university medical center. Results: We have found nursing care issues before thrombolytic treatment. Those were resolved with successful thrombolytic treatment. Following nursing issues were found: irregular breathing patterns, impaired self-care, reduced ability for personal hygiene, difficulty in swallowing with the risk of an aspiration, uncontrolled urinary and bowel functions, obstipation, impaired verbal communication and risk of falling. The disability of the patient was significantly reduced after thrombolysis. Conclusions: In stroke thrombolysis early arrival to the qualified center is essential. A nurse with overview of the nursing issues in stroke patients is an important member of the stroke team. Typical nursing care issues should be recognized, evaluated, followed and managed to reduce their impact on the patient.
