Abstract

Teoretična izhodišča in namen: V diplomskem delu smo predstavili duševno motnjo depresijo, ki je najpogostejša duševna motnja v vseh starostnih obdobjih. Klinična slika depresivne motnje je zelo heterogena, saj ima specifične značilnosti, ki so odvisni predvsem od spola in starosti obolelih oseb. Epidemiološke značilnosti, etiologija in klinična slika depresije pri starostnikih so predstavljene z namenom, da prispevamo k boljšemu razumevanju vpliva depresivne motnje na kakovost življenja starostnikov. Raziskovalne metode: V raziskavo smo vključili 138 starostnikov – stanovalcev izbranih domov za starejše občane v Pomurju. Polovica vseh anketiranih starostnikov je imela diagnosticirano depresivno motnjo ali so prejemali antidepresivno terapijo, druga polovica anketiranih pa ne. Raziskava je temeljila na primerjavi kakovosti življenja dveh skupin starostnikov s pomočjo vprašalnika EuroQol EQ-5D (Euro Quality of Life – 5 Dimension), ki je namenjen ocenjevanju kakovosti življenja v povezavi z zdravstvenim stanjem. Vprašalnik je sestavljen iz petih vprašanj, ki se nanašajo na področja okretnosti, osebne nege, običajnih dejavnosti, bolečine in depresije oziroma zaskrbljenosti ter vizualno-analogne skale. Rezultati: Ugotovili smo, da je kakovost življenja depresivnih stanovalcev značilno slabša (p < 0,001) kot kakovost življenja stanovalcev brez depresije. Stanovalci z depresijo se značilno manj (p < 0,005) vključujejo v družabne aktivnosti kot stanovalci brez depresije. Prav tako je samoocena zdravja anketiranih stanovalcev z depresijo, prikazana z vizualno-analogno skalo, značilno slabša (p < 0,001) kot pri stanovalcih brez depresije. Sklep: Raziskava dokazuje potrebo po nenehnem raziskovanju in spremljanju kakovosti življenja starostnikov, še zlasti tistih v domovih za starejše. Le na osnovi izsledkov je mogoče načrtovati in izvajati ukrepe za izboljšanje kakovosti življenja starejših. Predvsem je pomembno, da zaposleni v domovih za starejše poznajo značilnosti depresije v starostnem obdobju in načrtujejo ustrezne aktivnosti. Ohranjati je potrebno posameznikov občutek lastne učinkovitosti in zadovoljstva ter ohranjati in spodbujati socialne stike z družino, prijatelji in znanci. Prav tako je potrebna vključitev prostovoljnega dela s starostniki in medgeneracijsko sodelovanje, ki je v tujini zelo dobro razvito in ima dokazane pozitivne učinke. Izboljšanje kakovosti življenja stanovalcev domov za starejše lahko zagotovimo s celovitim individualnim pristopom.

Secondary abstract: Theoretical background and purpose: In this diploma work we presented a mental disorder called depression, which is also the most common mental disorder in all ages. Clinical picture of depression disorder is very heterogeneous and has its specific characteristics dependent on the sex and age of affected persons. Presented are the epidemiological characteristics, etiology and clinical picture of depression among elderly with the purpose to contribute to better understanding of the influence of depression disorder on the quality of life of elderly. Research methods: Research work included 138 elderly people, who live in chosen residential homes for senior citizens in Pomurje. Half of the respondents had a diagnosis of depression disorder or were receiving anti-depression therapy and the other half of the respondents not. The research was based on comparison of the life quality of two groups of elderly people with help of EuroQol EQ-5D (Euro Quality of Life – 5 Dimension) questionnaire, which is intended for the assessment of health status related quality of life. The questionnaire consists of five questions from the fields of agility, personal care, usual activities, pain and depression/anxiety and visual analog scale. Results: We ascertained that the quality of life of depressed tenants is significantly worse (p < 0,001) than the quality of life of tenants without depression. Tenants with depression are significantly less (p < 0,005) including themselves in social activities, than the tenants without depression. Likewise the self reported health status, reported with help of visual analogue scale, is significantly worse (p < 0,001) than with the tenants without depression. Conclusion: This research proves the need for ongoing research work and monitoring of quality of life of elderly, especially those who live in residential homes for senior citizens. Planning and implementations of actions for improvement of quality of life of elderly can be carried out only on the basis of research findings. Particularly important is that the employees of residential homes are well aquatinted with characteristics of depression among elderly and plan appropriate activities. It is necessary to maintain every individual's sense of its own effectiveness and satisfaction. Social contacts with family, friends and acquaintances have to be maintained and encouraged. Volunteer work with elderly and intergenerational cooperation, which is very well developed and proved to have positive effects abroad, also has to be included. The improvement of quality of life of tenants of residential homes for elderly can be ensured with holistic individual approach.
