Abstract
Medicinske sestre imajo pri svoji poklicni poti zelo pomembno nalogo. Pogosto se srečujejo s težko bolnimi in umirajočimi. Kako takrat postopati, da se ohrani dostojanstvo v teh zadnjih dneh in omogočiti dostojno smrt? Zdravstvena nega zajema široko področje obravnave pacientov. Medicinska sestra mora poznati vsa ta področja in jih vključiti v vsakdanje delo. Seznanjena mora biti z vsemi pravnimi in etičnimi vidiki zdravstvene nege, s katerimi se srečuje pri svojem delu. Namen magistrskega dela je predstaviti pravne in etične podlage, ki bi omogočile težko bolnim in umirajočim lažje in dostojno slovo. Ugotoviti želimo odnos medicinskih sester do umirajočega, njihovo seznanjenost oziroma poučenost upoštevanja temeljnih človekovih pravic in njihovo vključevanje v prakso.
Raziskovalna metodologija. Uporabljena je bila kvantitativna metodologija raziskovanja. Podatke smo zbrali s tehniko delno strukturiranih vprašalnikov za 100 zaposlenih v zdravstveni negi, potekala pa je v Univerzitetnem kliničnem centru Maribor. Anketni vprašalnik je bil sestavljen na podlagi Amsterdamske deklaracije o promociji pacientovih pravic v Evropi in mnenja Evropskega socialnega odbora o pacientovih pravicah.
Rezultati: Rezultati raziskave so pokazali, da medicinske sestre slabo poznajo pravice neozdravljivo bolnega ali umirajočega, kljub temu, da se z njimi pogosto srečujejo. Tega se zavedajo in so mnenja, da imajo premalo tako teoretičnega kot praktičnega znanja, ki pa si ga želijo pridobiti. Umirajočemu niso zagotovljene pravice po Amsterdamski deklaraciji, saj teh pravic večina ne pozna. Zaposleni v zdravstveni negi poznajo program hospic, vendar so prepričani, da se ga premalo vključuje v vsakdanje okolje. Statistično smo ugotovili, da bolj kot anketirani menijo, da imajo potrebno znanje za delo z neozdravljivo bolnimi, bolje poznajo program hospic in vsebino Amsterdamske deklaracije. Prav tako smo ugotovili statistično značilno razliko med izobrazbo in poznavanjem vsebine Amsterdamske deklaracije. Med višje izobraženimi je poznavanje boljše.
Sklep: Zdravstveni delavci so dolžni omogočiti težko bolnim in umirajočim dostojno smrt v okviru svojih kompetenc. Vendar morajo v osnovi poznati vse pravice, ki iz tega izhajajo. Medicinske sestre te pravice slabo poznajo, zato tem področjem ne posvečajo dovolj pozornosti. Potrebno bi bilo informirati tako paciente, ki bi se morali zavedati svojih pravic in biti o njih tudi seznanjeni, kot tudi izobraževati medicinske sestre.

Nurses have a very important task in their career. They are often faced with the severely sick and dying people. We should ask ourselves how to act in such cases in order to maintain the dignity of a sick person and let him/her die with dignity? A nurse should know all these areas and include them in their daily routine in dealing with people. She should be informed about all legal and ethical aspects of health care. The purpose of our Master's thesis is to present the legal and ethical basis that would help sick and dying patients to die with dignity. We want to determine the kind of nurses’ attitude towards a dying patient, their knowledge and awareness of the fundamental human rights and their integration into practice.
Research Methodology: We used the quantitative research method. Data were collected with partially structured questionnaires filled out by 100 employees in nursing at the University Medical Centre Maribor. The questionnaire was compiled on the basis of the Amsterdam Declaration on the Promotion of Patients' Rights in Europe and on the opinion of the European Social Committee on the Patient’s Rights.
Results: The results showed that nurses are unfamiliar with the rights of the terminally ill or dying patients, despite the fact that they encounter such situations at work. They are aware of lack of their both theoretical and practical knowledge, but they are willing to improve these skills. Amsterdam Declaration rights are not guaranteed for the dying patient because most nurses do not know the content of the rights. Employees in health care system know the hospice program, but are certain that it is insufficiently integrated into the everyday environment. Statistically, we found out that the more knowledge the people have about dealing with the terminally ill, the more they know the hospice program and the content of the Amsterdam Declaration. We also found a statistically significant difference between education and knowledge of the contents of the Amsterdam Declaration. The knowledge of the Amsterdam Declaration is better among the nurses with a higher education.
Conclusion: Health professionals are obliged, within their competences, to enable the severely ill and dying patients to die with dignity. What emerges from this is that they need to have a basic knowledge about all rights of the dying patients. Nurses do not have enough knowledge about these rights therefore they do not pay enough attention to this area. It would be necessary to inform the patients about these rights to become aware of them, and to educate the nurses. |
