Abstract

Referenčne ambulante so ambulante družinske medicine, kjer se je srednji medicinski sestri pridružila še diplomirana medicinska sestra, ki izvaja presejalne preglede srčno - žilnih bolezni, v skladu s protokoli odkriva in obravnava paciente s kroničnimi boleznimi, s poudarkom na promociji zdravja in zdravim življenjskim slogom. Za zgodnje odkrivanje kroničnih bolezni, so zelo pomembni preventivni pregledi, ter svetovanje o le teh. V referenčni ambulanti ima diplomirana medicinska sestra pomembno vlogo pri preventivi in spremljanju urejene kronične bolezni.

Secondary abstract:	Reference practices are practices of family medicine, where the middle nurse is joined by the graduated nurse, who caries out screening of cardio - vascular diseases and in accordance with protocols discovers and treats patients with chronic diseases with emphasis on the promotion of heaath and a healthy lifestyle. For early discovery of chronic noncommunicable diseases are very important preventive examinations and consultations about them. In the reference practice, the graduatet nurse has an important role in the prevention and monitoring of the chronic disease.
